護士：我們3個人坐在一起談話的目的是一致的，這就是幫助和配合醫(yī)生治好病人的盆腔炎癥，盡快懷孕。

病人丈夫：是的，我也這么想（婆婆同時點點頭）。

護士：但是，對不孕的原因你們二位可能還缺乏認識?，F(xiàn)在對病人來說，除了盆腔自身的炎癥外，心理緊張和壓力也是主要原因，因為任何緊張都可使病人內(nèi)分泌系統(tǒng)紊亂，女性激素分泌失調(diào)，排卵受抑等等。所以我們目前需要共同努力的是：多給病人關(guān)心愛撫和支持，盡可能不提生育之事，讓她以完全放松的狀態(tài)接受藥物治療。盆腔炎癥不是難治之癥，只要好好與醫(yī)生合作，會治愈的。

病人婆婆：是嗎？那么需要多長時間？

護士：最長3個月。如果是慢性、有粘連的，出院后還要堅持做一段時間理療。

病人丈夫：做理療對懷孕有副作用嗎？

護士：一般沒有。

病人婆婆：好的，從現(xiàn)在起，我和我兒子都不再提生育的事，一心一意為她治病。

病人丈夫：是的，我們會盡力幫助她減輕壓力的，謝謝你，護士。

Interactions

Nurse：I believe that we all have the same objective which is to assist and cooperate with the doctor to treat Mrs.Yang’s pelvic inflammation so that she may become pregnant soon.

Patient’s Husband：Yes，I think so.（The mother－inlaw nodded too）

Nurse：You may not fully understand the reasons of not been able to pregnant.In addition to the pelvic inflammation，the major reasons for her not become pregnant，are possible related to mental stress and nervousness.Nervousness can mess－up her endocrine system and secretion of female hormones which might suppress the ovulation.What we must do now is to give the patient more care，love and support，and don’t keep mention about pregnancy.Let’s create a relaxed atmosphere for her and at the same time let her receive medical treatment.I should say that the pelvic inflammation is curable if we cooperate with the doctor.

Patient’s Mother－In－Law：Really？How long will it take？

Nurse：No more than three months.If her disease is chronic，and has adhesion，then physiotherapy may be needed for some time after her discharge.

Patient’s Husband：Does physiotherapy has any side effects on pregnancy？

Nurse：Generally speaking there is no side effect.

Patient’s Mother－In－Law：Well，from now on my son and I will not going to say a word about child－bearing any more.We will help her to receive the treatment with all our hearts.

Patient’s Husband：Yes.We’ll do our best to reduce her pressure.Thank you，nurse.

在護士的指導(dǎo)、家屬的關(guān)心、支持、醫(yī)生的積極治療下，病人很快就康復(fù)了。半年后，病人丈夫楊某打電話找到責(zé)任護士，向她報告了妻子懷孕一個月的喜訊并轉(zhuǎn)達了全家對她的謝意。

Result

The patient recovered quickly under the nurse’s instructions，the care and support of her family members，and the aggressive treatment of the doctor.Six month later，the patient’s husband phoned the nurse and told her that his wife had become pregnant for a month and thanked the nurse for all her help.

不孕的婦女心理壓力較大，尤其是來自家庭社會方面的壓力。文中護士運用了較好的目標轉(zhuǎn)移溝通技巧，通過對不孕者的移情性理解，對其婆婆丈夫進行不孕原因的分析、告知和有效疏導(dǎo)，幫助個案松解壓力。有效地運用了社會支持系統(tǒng)，從根本上減少了不孕的心理壓力，最終取得圓滿護理效果。

Comments

Infertile women have high mental pressure，especially from her family and the society.In this case，the nurse utilized the object－transferring communication skill，she understands the patient’s emotion－transfer phenomenon of infertile woman，effectively guided and explained the cause of infertility to her mother－in－law and her husband，and used social supporting system effectively.It reduced the mental stress of infertility.The nursing outcome was very satisfactory.
