針藥結合治療混合痔術后疼痛40例臨床觀察

【摘 要】 目的：觀察承山、中髎、下髎穴埋針治療混合痔術后疼痛的有效性及安全性，為混合痔術后疼痛的緩解尋求可行性方法。方法：選取混合痔外剝內扎術后患者80例，隨機分成對照組和治療組，術后常規治療，對照組40例口服酮咯酸氨丁三醇膠囊鎮痛治療，治療組40例在此基礎上應用“清鈴”牌撳針進行貼壓鎮痛;體內留針24h，每日治療1次，同時囑患者不定時按壓所貼穴位，術后1～5日每天記錄患者排便和換藥時的疼痛程度以及疼痛持續時間，采用數字評分法（NRS）值評價療效;比較兩組的鎮痛效果及不良反應。結果：對照組與治療組術后5d疼痛評分相比較，治療組顯著低于對照組（P<0.05），具有統計學意義;伴隨著治療次數的增加，兩組患者的疼痛評分值也相繼降低，治療組的術后止痛效果較對照組穩定而持久，兩組間比較差異具有統計學意義（P<0.05）。對照組和治療組的總有效率分別是85 %和92.5%，兩組比較差異無統計學意義（P>0.05）。結論：混合痔術后采取承山、中髎、下髎穴撳針埋針結合酮咯酸氨丁三醇膠囊口服效果滿意。

Abstract：Objective To observe the efficacy and safety of embedding needles at Chengshan， Zhongliao and Xialiao points in the treatment of post-operative pain of mixed hemorrhoids， and to find a feasible method for relieving post-operative pain of mixed hemorrhoids.Methods 80 patients with mixed hemorrhoids after external stripping and internal ligation were randomly divided into control group and treatment group. Routine treatment was given after operation. 40 patients in control group were given oral ketorolac tromethamine capsule for analgesia. 40 patients in treatment group were given “Qingling” brand acupuncturing needle for pressing and analgesia on this basis. The needle was retained in vivo for 24 hours， once a day. At the same time， the patients were instructed to press the place indefinitely. The pain degree and duration during defecation and dressing change were recorded every day from 1 to 5 days after operation. The curative effect was evaluated by NRS value. The analgesic effect of the two groups was compared and the adverse reactions of the treatment were observed.Results Compared with the treatment group， the pain score of the treatment group was significantly lower than that of the control group （P<0.05）， with statistical significance; with the increase of the number of treatments， the pain score of the two groups also decreased. The analgesic effect of the treatment group was more stable and lasting than that of the control group， and there was significant difference between the two groups （P<0.05）. The total effective rates of the control group and the control group were 85% and 92.5%， respectively. There was no significant difference between the two groups （P>0.05）. Conclusion Chengshan， Zhongliao and Xiaqiao acupoints needle embedding combined with ketorolactic acid ambutriol capsule is effective for mixed hemorrhoids after operation.
