認(rèn)知行為干預(yù)護(hù)理在20~30歲腹膜透析患者中的應(yīng)用及對(duì)應(yīng)對(duì)反應(yīng)和心理韌性的影響

【摘要】 目的：探討認(rèn)知行為干預(yù)護(hù)理在20～30歲腹膜透析（PD）患者中的應(yīng)用及對(duì)應(yīng)對(duì)反應(yīng)和心理韌性的影響。方法：選取2016年3月-2018年3月在筆者躲在醫(yī)院進(jìn)行PD治療的中青年患者84例為研究對(duì)象，采用隨機(jī)數(shù)表法分為干預(yù)組和對(duì)照組，各42例，對(duì)照組在透析過(guò)程中給予常規(guī)護(hù)理，干預(yù)組在此基礎(chǔ)上給予認(rèn)知行為干預(yù)護(hù)理，比較兩組干預(yù)后效果及對(duì)患者應(yīng)對(duì)反應(yīng)和心理韌性的影響。結(jié)果：干預(yù)后觀察組屈服應(yīng)對(duì)及回避應(yīng)對(duì)得分顯著降低，面對(duì)應(yīng)對(duì)得分顯著升高，且觀察組屈服應(yīng)對(duì)及回避應(yīng)對(duì)得分低于對(duì)照組，面對(duì)應(yīng)對(duì)得分高于對(duì)照組（P<0.05），對(duì)照組干預(yù)后三種維度得分比較差異無(wú)統(tǒng)計(jì)學(xué)意義（P>0.05）;干預(yù)后兩組CD-RISC、QOL評(píng)分均升高，且觀察組干預(yù)后CD-RISC、QOL評(píng)分顯著高于對(duì)照組（P<0.05）。結(jié)論：認(rèn)知行為護(hù)理干預(yù)可顯著提高中青年P(guān)D患者應(yīng)對(duì)疾病的能力及心理韌性，并改善其生活質(zhì)量，值得在臨床上

推廣。

【Abstract】 Objective：To investigate the application of cognitive behavioral intervention in patients aged 20-30 years undergoing peritoneal dialysis （PD） and the effect on coping response and resilience.Method：A total of 84 young and middle-aged patients who underwent PD in the hospital from March 2016 to March 2018 were selected.They were divided into the intervention group and the control group by random number table method.The control group was given routine nursing during the dialysis.On the basis of routine nursing，the intervention group was gaven cognitive behavioral intervention.The intervention effects，effect on coping responses and resilience were compared between the two groups.Result：After the intervention，the scores of yielding response and avoidance response of the observation group were significantly decreased，and were lower than those of the control group while the facing response score was significantly increased and the was higher than that of the control group （P<0.05）.There were no significant differences in the scores of the three dimensions of the control group before and after intervention （P>0.05）.The CD-RISC and QOL scores of both groups were increased after the intervention，and the scores of the observation group were significantly higher than those of the control group （P<0.05）.Conclusion：Cognitive behavioral intervention can significantly improve the ability and resilience of young and middle-aged patients with PD to cope with diseases and improve their quality of life.
