磁性護理指導下的心理干預護理在斷指再植中的應用

【摘要】 目的：探討磁性護理指導下的心理干預護理在斷指再植中的應用效果。方法：選取2016年1月-2017年12月在筆者所在醫院手外科行斷指再植術109例患者為研究對象，按有無實施磁性護理指導下的心理護理干預分為觀察組及對照組。手術結束后第2天進行損害及殘疾的反應量表（RIDI），出院時進行創傷后應激癥狀量表（SPTSS）及住院焦慮及抑郁測量表（HADS）調查，比較兩組評分差異。結果：RIDI評分中，觀察組患者的沖擊、焦慮、否認、抑郁、內在化憤怒、外在化敵意6個項目評分低于對照組，而承認及調節2個項目評分高于對照組，差異均有統計學意義（P<0.05）。同時，觀察組患者出現創傷后應激障礙癥狀少于對照組，住院期間觀察組患者焦慮及抑郁評分亦低于對照組，差異均有統計學意義（P<0.05）。結論：磁性護理指導下的心理干預護理有利于調動患者自主性、發揮護理人員能動性，從而提高患者的依從性及自信心、減少不良心理事件發生。

【Abstract】 Objective：To explore the effect of psychological intervention nursing care guided by magnetic nursing in digital replantation.Method：From January 2016 to December 2017，109 patients who underwent replantation of severed fingers in our hospital were selected as the study subjects.According to whether or not psychological care interventions were conducted，the patients were divided into observation group and control group.On the second day after the completion of the operation，a survey on the scale of the reactions to impairment and disability inventory（RIDI） was performed.After discharge，the posttraumatic stress symptoms scale（SPTSS） and the hospital anxiety and depression scale（HADS） survey were conducted.Comparison of the differences of scores between the two groups.Result：In the RIDI score，the scores of the 6 items in the observation group were lower than the control group in the impact，anxiety，denial，depression，internalized anger and externalization of hostility，while the scores of the 2 items in the admission and adjustment were higher than those in the control group（P<0.05）.At the same time，the symptoms of posttraumatic stress disorder in the observation group were less than those in the control group，and the anxiety and depression scores of the subjects in the observation group were also lower than those in the control group during the hospitalization（P<0.05）.Conclusion：The psychological intervention nursing under the guidance of magnetic nursing is helpful to mobilize the patients autonomy and give play to the mobility of the nursing staff，so as to improve the patients compliance and self-confidence and reduce the incidence of adverse psychological events.
