前列腺癌根治手術后尿失禁患者施予優質護理干預的價值及效果觀察

【摘要】 目的：對前列腺癌根治手術后尿失禁患者施予優質護理干預的價值及效果進行觀察評價。方法：選取2015年6月-2018年5月于筆者所在醫院接受根治術治療的前列腺癌患者56例為研究對象，以隨機數字表法將其分為優質化施護組與常規化施護組，每組28例，常規化施護組以前列腺癌常規圍術期護理方案施護，術后尿失禁防控主要為于術后尿管拔除后行肛提肌訓練指導，優質化施護組施以針對術后尿失禁防控的優質化護理管理，比較兩組患者術后尿失禁發生率、尿失禁持續時間、尿失禁專項防控護理認可度。結果：優質化施護組干預后的尿失禁發生率與尿失禁持續時間均顯著優于常規化施護組，尿失禁專項防控護理的認可度評分顯著較常規化施護組高，差異均有統計學意義（P<0.05）。結論：將優質化施護模式應用實踐于前列腺癌根治術患者圍術期護理中，可實現可靠度較高的尿失禁防控管理效果，深受患者高度認可。

【Abstract】 Objective：To observe and evaluate the value and effect of quality nursing intervention for urinary incontinence patients after radical prostate cancer surgery.Method：A total of 56 patients with prostate cancer who received radical prostatectomy in our hospital from June 2015 to May 2018 were selected as research objects.By random number table method，they were divided into the high-quality nursing group and the routine nursing group，with 28 cases in each group.The routine nursing group used the routine perioperative nursing scheme for prostate cancer.The prevention and control of urinary incontinence after operation was mainly guided by levator ani muscle training after the removal of urinary catheter.The high-quality nursing group was given high-quality nursing management for prevention and control of urinary incontinence after operation.The incidence of urinary incontinence，duration of urinary incontinence，recognition of special prevention and control nursing for urinary incontinence were compared between the two groups.Result：The incidence of urinary incontinence and duration of urinary incontinence in the high-quality nursing group were significantly better than those in the routine nursing group，and the recognition score of special prevention and control nursing for urinary incontinence was significantly higher than that in the routine nursing group，the differences were statistically significant（P<0.05）.Conclusion：The application of high quality nursing model in perioperative care of patients with radical prostatectomy can achieve a high reliability of urinary incontinence prevention and control management effect，which is highly recognized by patients.

前列腺癌是老年男性常見的泌尿生殖系統腫瘤類型，且處于發病率逐年走高狀態，前列腺癌根治術為該類腫瘤的主流化首選疾控技術，但接受根治術后有高達60%的受術者會受尿失禁問題困擾，在生理心理社會等多個層次與角度對患者形成痛苦折磨，故而對前列腺癌根治術者施以術后尿失禁積極防控管理至關重要，在術時謹慎操作嚴防盆底肌肉損傷并行膀胱頸部重建的同時，亦需于根治術圍術期提供高質高效的專項尿失禁防控護理，如此方能提高前列腺癌根治術受術者的手術獲益度[1-3]。本研究嘗試將優質化施護模式應用實踐于前列腺癌根治術患者圍術期護理中，得到較高的護理見效度，現報告如下。
