MMP-2蛋白及E-cad蛋白在胰腺癌組織中的表達及臨床意義探索和研究

目的 探討MMP-2蛋白及E-cad蛋白在胰腺癌組織中的表達及臨床意義。方法 收集2011年12月至2013年1月本院確診的胰腺癌組織標本30例設為觀察組，收集同期正常胰腺組織10例設為對照組，采取免疫組織化學法對兩組胰腺組織中MMP-2、E-cad的表達情況進行觀察。結果 觀察組MMP-2蛋白陽性率為93.33%，E-cad蛋白陽性率為96.67%；對照組MMP-2蛋白陽性率為60%，E-cad蛋白陽性率為20%，觀察組的MMP-2蛋白陽性率及E-cad蛋白陽性率均顯著高于對照組，P＜0.05，有統計學意義；MMP-2蛋白、E-cad蛋白陽性表達和胰腺癌的分化程度有一定關聯；MMP-2蛋白、E-cad蛋白陽性表達和胰腺癌是否存在淋巴結轉移有一定關聯。結論 MMP-2蛋白在胰腺癌組織中呈高表達，E-cad蛋白在胰腺癌組織中呈低表達，且二者表達情況和胰腺癌組織的分化程度以及是否存在淋巴結轉移均有密切關聯。

胰腺癌為惡性程度較高的腫瘤性疾病，臨床診斷、治療都具有一定困難[1]，隨著分子生物學的進一步發展，臨床對于MMP-2、E-cad蛋白和惡性腫瘤之間的關系進行了一系列的研究。為進一步探討MMP-2蛋白及E-cad蛋白在胰腺癌組織中的表達及臨床意義。本文特收集2011年12月至2013年1月本院確診的胰腺癌組織標本30例及同期正常胰腺組織10例相關資料進行比較性分析，現將研究情況進行如下報道。

1 資料與方法

1.1一般資料：收集2011年12月至2013年1月本院確診的胰腺癌組織標本30例設為觀察組，收集同期正常胰腺組織10例設為對照組。其中觀察組樣本來自男性21例，女性9例，年齡最小的21歲，最大的72歲，平均年齡為（43.23±2.12）歲。高度分化10例，中度分化15例，低度分化5例；存在淋巴轉移18例，不存在淋巴結轉移12例。對照組樣本來自男性7例，女性3例，年齡最小的20歲，最大的71歲，平均年齡為（42.12±2.09）歲。觀察組標本來源于胰腺癌手術切除后病理送檢樣本，對照組標準來源于腹部外傷或者胰腺良性病變手術所切除后病理送檢樣本。

1.2 方法：采取免疫組織化學法對兩組胰腺組織中MMP-2、E-cad的表達情況進行觀察。將所有樣本使用濃度為10%的甲醛液進行固定，石蠟包埋，切4 μm片，染色觀察[2]。

1.3 陽性表達判斷標準

1.3.1 MMP-2：陽性情況為在胞質、胞膜存在明顯棕黃色顆粒狀染色。強陽性：細胞陽性率高于70%；陽性：細胞陽性率處于50%～70%；弱陽性：細胞陽性率處于30%～50%；陰性：細胞陽性率低于30%[3]。MMP-2表達陽性率=（強陽性例數+陽性例數）/總例數×100%。

1.3.2 E-cad陽性：腫瘤細胞著色強度同正常黏膜上皮相同，陽性細胞超過75%；陰性：腫瘤細胞較正常黏膜上皮著色強度弱或者陽性細胞低于75%[4]。E-cad陽性表達率=陽性例數/總例數×100%。

1.4 統計學方法：應用SPSS13.0軟件進行統計分析，計量資料比較采用t檢驗，計數資料比較采用χ2檢驗，P＜0.05為差異有統計學意義。

2 結 果

2.1 兩組胰腺組織MMP-2蛋白、E-cad蛋白陽性表達情況：觀察組MMP-2蛋白陽性率為93.33%，E-cad蛋白陽性率為96.67%；對照組MMP-2蛋白陽性率為60%，E-cad蛋白陽性率為20%，觀察組的MMP-2蛋白陽性率及E-cad蛋白陽性率均顯著高于對照組，P＜0.05，有統計學意義，見表1。

2.2 不同分化程度MMP-2蛋白、E-cad蛋白陽性表達情況：MMP-2蛋白、E-cad蛋白陽性表達和胰腺癌的分化程度有一定關聯，見表2。

Objective To investigate the expression and clinical significance of MMP-2 and E-cad protein in pancreatic tissue. Methods December 2011 January 2013 hospital diagnosed 30 cases of pancreatic tissue observation group to collect the same period 10 cases of normal pancreatic tissue as control group, taking two groups of immunohistochemistry pancreatic tissue MMP-2, E-cad expression was observed. Results Observation group MMP-2 protein positive rate of 93.33%, E-cad protein positive rate was 96.67%; controls MMP-2 protein positive rate 60%, E-cad protein positive rate was 20% in the observation group MMP-the positive rate of E-cad 2 protein and protein -positive rate was significantly higher, P＜0.05, statistically significant ; MMP-2 protein, E-cad expression and differentiation of certain proteins associated with pancreatic cancer; MMP- 2 protein , E-cad protein expression and pancreatic lymph nodes if there is a certain association. Conclusion MMP-2 protein was highly expressed, E-cad protein expression was lower in pancreatic cancer in pancreatic cancer, and both the expression and the degree of differentiation of pancreatic cancer, and whether the presence of lymph node metastases are closely related.
