白色白內(nèi)障超聲乳化手術(shù)撕囊技巧

【摘要】 目的：探討白色白內(nèi)障超聲乳化手術(shù)的撕囊方法。方法：對(duì)76例76只眼，應(yīng)用一種手法撕囊。向前房?jī)?nèi)注入黏彈劑，在上方用截囊針截開晶狀體前囊約2 mm，用囊膜剪在晶狀體前囊截囊口兩端呈鈍角剪開前囊膜約2～3 mm，再用撕囊鑷夾持前囊右側(cè)角逆時(shí)針完成撕囊。結(jié)果：76只眼白色白內(nèi)障應(yīng)用此方法環(huán)形撕囊均獲成功，成功率為100%，超聲乳化手術(shù)順利完成。結(jié)論：在白色白內(nèi)障超聲乳化手術(shù)中，應(yīng)用截囊聯(lián)合剪切撕囊簡(jiǎn)便、安全、有效，其可顯著提高白色白內(nèi)障連續(xù)環(huán)形撕囊的成功率，便于掌握和應(yīng)用。

【Abstract】 Objective： To find a way of capculorhexis in phacoemulsification for patients with white cataract. Method： Phacoemulsification was performed in seventy-six eyes of 76 patients with white cataract using a capculorhexis technique. Viscoelastic was injected to fill anterior chamber. Firstly， a cut was made on the anterior capsule about 2 mm length using discission needle. Cutting two sides of the anterior capsule opened in an obtuse angle with scissors and the continuous curvilinear capsulorhexis （CCC） was finished. Result： All cases with white cataract had successful results of CCC （100%）. Conclusion： In the white cataract phacoemulsification the use of combinational capculorhexis technique is an easy， safe and effective method. It significantly improves the success rate of CCC. And it is easy to handle and extend.

白內(nèi)障超聲乳化手術(shù)相對(duì)于傳統(tǒng)的白內(nèi)障囊外摘除術(shù)而言，具有手術(shù)切口小、組織損傷輕、安全性好、恢復(fù)快、無(wú)需等待白內(nèi)障成熟才施行手術(shù)等優(yōu)點(diǎn)在臨床得到廣泛開展。而連續(xù)環(huán)形撕囊術(shù)是手術(shù)順利完成的關(guān)鍵步驟之一[1]。隨著我國(guó)人口老齡化進(jìn)程加快，白內(nèi)障患者不斷增多，其中白色白內(nèi)障患者占有相當(dāng)比例。白色白內(nèi)障是指晶狀體皮質(zhì)白色混濁或晶狀體核硬化混濁，在手術(shù)顯微鏡下缺乏眼底紅光反射，以致難以區(qū)分晶狀體前囊膜和皮質(zhì)的白內(nèi)障[2]。由于眼底紅光反射缺失，前囊膜與其下方皮質(zhì)的對(duì)比度差，撕囊過(guò)程中不易辨認(rèn)撕囊軌跡，易向周邊部撕裂而不易被發(fā)現(xiàn)，導(dǎo)致撕囊失敗，影響后續(xù)手術(shù)操作[1]。為解決白色白內(nèi)障撕囊困難的難題，在白色白內(nèi)障超聲乳化手術(shù)中，筆者應(yīng)用截囊聯(lián)合剪切兩側(cè)囊膜再撕囊的方法，顯著提高了白色白內(nèi)障連續(xù)環(huán)形撕囊的成功率，取得了滿意的效果。……
