小兒肛周膿腫手術(shù)時機(jī)探討

[摘要] 目的 研究手術(shù)時機(jī)對小兒肛周膿腫治療效果的影響。方法 抽取2013年8月—2014年8月在該院肛腸科就診的86例小兒肛周膿腫患兒進(jìn)行臨床研究，根據(jù)手術(shù)時機(jī)的不同分為觀察組與對照組，觀察組患兒在發(fā)病一期進(jìn)行根除手術(shù)，對照組患兒在發(fā)病一期后進(jìn)行根除手術(shù)，觀察比對兩組患兒的手術(shù)治療效果及并發(fā)癥。 結(jié)果 經(jīng)治療后，觀察組患兒的治療有效率（100.0%）明顯好于對照組（58.1%），復(fù)發(fā)率（0.0%）也顯著低于對照組（23.1%），差異有統(tǒng)計學(xué)意義（P<0.05）；治療結(jié)束后對患兒及家屬進(jìn)行滿意度調(diào)查，觀察組滿意度（100.0%）明顯高于對照組（60.5%），差異有統(tǒng)計學(xué)意義（P<0.05）。結(jié)論 合適的手術(shù)時機(jī)對小兒肛周膿腫具有重要意義，合適的手術(shù)時機(jī)可有效提高患者的存活率，提高治愈率，降低復(fù)發(fā)率和并發(fā)癥的發(fā)生率，具有可靠的臨床意義，可在臨床治療中廣泛推廣。

[Abstract] Objective To study the effect of timing of surgery on the treatment effect of perianal abscess in children. Methods selected from 2013 August --2014 year in August to conduct a clinical study of 86 cases of pediatric anus week abscess in children with anorectal treatment in our hospital， according to the different timing of surgery of the observation group and the control group， the observation group with the onset of a period in radical surgery， the control group were performed radical surgery in the pathogenesis of a period after the observation of surgical treatment the effect and complications of two groups of children with alignment. Results after the treatment， the observation group with the treatment efficiency（100%） was significantly better than the control group （58.1%）， the recurrence rate （0%） was significantly lower than that of the control group （23.1%）， with significant difference （P<0.05）； after the end of treatment satisfaction survey conducted on children and families， the observation group satisfaction （100%） obviously higher than that of the control group （60.5%）， with significant difference （P<0.05）. Conclusion proper operation time has the important meaning to the children of perianal abscess， proper operation time can effectively improve the survival rate of patients， improve the cure rate and reduce the recurrence rate and the incidence of complications， clinical significance and reliable， can be widely used in clinical treatment.

小兒肛周膿腫是常見的一種肛腸科疾病[1]，造成原因多有免疫力低下、先天性肛瘺、全身或鄰近肛門處化膿性感染、肛竇炎等[2]，常見體征有肛門直腸周圍紅腫脹痛、有壓痛、觸摸會痛，臨床癥狀有肛門周圍疼痛腫脹、伴隨發(fā)熱感染等[3]全身癥狀。小兒肛周濃重具有發(fā)病急、病變快等特點(diǎn)[4]，高熱容易引發(fā)患兒發(fā)展成為敗血癥[5]，因此及時有效地治療對于挽救患兒的生命具有重要意義。該研究為探究手術(shù)時機(jī)對小兒肛周膿腫治療效果的影響，抽取2013年8月—2014年8月在該院就診的86例小兒肛周膿腫患兒進(jìn)行臨床研究，取得滿意效果，現(xiàn)報道如下。……
