大型綜合醫院日間手術患者雙向轉診的研究

摘要：目的 探索大型綜合醫院日間手術病人雙向轉診的方法。方法 大型綜合醫院、基層醫院領導、醫生須認真學習雙向轉診的相關文件，掌握雙向轉診的原則、步驟、流程，建立醫院雙向轉診辦公室。醫生填寫雙向轉診單，就診時有雙向轉診單的患者可得到優先處理。結果 基層醫院需要做日間手術的患者能轉到大型綜合醫院，大型綜合醫院日間手術患者術后能轉到基層醫院，基層醫院幫助患者換藥，飲食指導、功能鍛煉等康復治療和護理。結論 大型綜合醫院日間手術患者雙向轉診的開通，為日間手術患者提供優質、安全、有效、方便、快捷、經濟的服務，為大型綜合醫院醫療資源的充分利用提供思路。對解決患者看病難、看病貴、住院難、手術遲有重要意義。

Abstract：Objective To explore the large-scale integrated method of two-way referral hospitalambulatory surgery patients .Methods Large general hospital ， hospital leadership at the grass-roots level ， the doctorshall earnestly study the bidirectional referral of relevant documents ， to master theprinciple of two-way referral ， steps ， processes ， and establish a two-way referral hospitaloffice . The doctor fill two-way referral form ， two-way referral sheet of the patients canget priority .Results Basic-level hospitals need do ambulatory surgery patients can go to the largegeneral hospital ， large general hospital ambulatory surgery patients postoperative can goto the grass-roots hospitals ， grass-roots hospital to help patients treatment ， dietguidance ， functional exercise rehabilitation treatment and nursing .Conclusion The opening of the large-scale integrated hospital ambulatory surgery patientstwo-way referral ， for ambulatory surgery patients to provide quality ， safe ， effective ，convenient ， fast ， economic services ， for the large general hospital for better utilization ofmedical resources . To solve the difficulty and high cost of patients ， hospital ， surgery later is important .

日間手術（day surgery）亦稱非住院手術，當日歸宅手術，是指手術患者在入院前做完檢查、麻醉評估，然后預約手術時間，當日住院，當日手術，24 h內出院的一種手術模式[1]。從20世紀80年代開始，日間手術模式在歐美國家得以迅猛發展，這主要得益于醫學的發展和人們認識的提高，同時與1995年有12個國家和地區的日間手術協會共同組建的國際日間手術協會（The internation association of ambulatory surgery，IAAS）的大力推廣不可分割[2]。20多年來，日間手術量在歐美國家穩步增長，近期很多國家日間手術量已占其擇期手術的60%以上。在丹麥、西班牙、瑞典等國家，分別達到89%、87%、和80%[3]。我國香港，20世紀90年代開始就已經開展日間手術，而內地則開展較晚[4]。……
