腸腔內、外雙套管負壓引流在低位直腸癌一期切除吻合術中的應用

目的探討腸腔內、外雙套管負壓引流在低位直腸癌一期切除吻合中的可行性與安全性。方法回顧性收集2009年1月—2014年12月期間收治的650例低位直腸癌患者資料,分為對照組(n=220)、造口組(n=205)和引流組(n=225)。對照組采用Dixon術(直腸低位前切除術);造口組行Dixon術加預防性末端回腸造口術,二期手術還納;引流組行Dixon術,術中放置腸腔內、外雙套管持續灌洗負壓引流。比較3組患者手術時間、術中出血量、術后排氣時間、住院費用的差異。術后進行隨訪,分析3組術后吻合口漏及術后感染等并發癥發生情況。結果3組手術時間、術中出血量以及術后排氣時間差異無統計學意義(P>0.05);引流組患者住院時間和住院費用低于造口組(P<0.05)。在并發癥方面,3組患者的吻合口漏發生率、切口感染、盆、腹腔感染、肺部感染的發生率差異無統計學意義(P>0.05)。術后隨訪結果顯示,3組患者的復發、轉移及死亡比例差異均無統計學意義(P>0.05)。結論腸腔內、外雙套管負壓引流可縮短低位直腸癌患者Dixon術后的住院時間,減輕患者負擔,能否降低術后吻合口漏發生率需進一步證實。

低位直腸癌是我國較為常見的惡性腫瘤,具有患病率高、患病時間早、腫瘤位置低的特點[1]。目前其手術治療主要采用吻合器輔助下的Dixon術式,但由于直腸特殊的解剖結構,吻合口張力大,血供不足,術后吻合口漏的發生率較高[2]。筆者近年來在行Dixon術時采用了腸腔內、外雙套管持續負壓吸引的方案,取得較好的臨床效果,現總結如下。
1.1 研究對象選取2009年1月—2014年12月間我院收治的650例直腸癌患者,按照手術方式分為對照組(220例)、造口組(205例)和引流組(225例)。納入標準:(1)入院前有便血、便頻、便細、黏液便等直腸癌臨床癥狀,影像學檢查提示直腸占位。(2)經腸鏡取材,病理學檢查確診為直腸癌。(3)接受一期切除吻合手術。排除標準:(1)全身情況差,惡病質狀態,不能耐受手術。(2)直腸癌位置距齒狀線<5 cm,腫瘤廣泛侵襲周圍組織,無法完全切除或已發生遠處轉移。(3)多發結、直腸腫瘤。所有入選的患者及其家屬均知情同意并簽署知情同意書。3組患者性別、年齡、腫瘤部位、TNM分期、白蛋白、血紅蛋白及白細胞計數差異均無統計學意義(P>0.05),見表1。

1.2 手術方法3組患者的手術均由同一專業組醫生施行。圍手術期治療:術前均行腸道準備,抗感染和營養支持治療,控制內科疾病。手術操作:所有病例均按照直腸全系膜切除(TME)原則進行手術,術中吻合一律應用吻合器或切割吻合器。對照組行傳統Dixon手術[3]:常規游離結腸系膜,牢固結扎腸系膜下動脈及伴行靜脈,分離并切除乙狀結腸和直腸,保證切緣距腫瘤>2 cm,且吻合口無張力。一期吻合,放置傳統單腔引流管。造口組[4]:Dixon術完成后行末端回腸造口術,一段末端回腸(3~5 cm)提拉至左下腹部皮膚表面,硬質塑料棒穿行于外置回腸下,防止其回縮。術后第2~ 3天切開外置回腸,引流其內容物。外置回腸回納的時間取決于患者臨床狀態,如無吻合口漏等術后并發癥,多在術后12 d進行。引流組[5]:Dixon術中放置腸腔內、外雙套管持續負壓引流。腫瘤所在直腸切除并吻合后,于距吻合口近端約10 cm的腸管處做一漿肌層荷包縫合。切開腸管,將自制的雙套引流管置入腸腔內,引流管前端跨過吻合口5 cm,收緊荷包線;引流管另一端引出體外,將引流管出腸壁處的漿膜與腹膜縫合固定,此為腸腔內雙套管引流,并在吻合口旁置入另一雙套引流管,為腸腔外雙套管引流,見圖1。雙套引流管均于手術完成后從腹壁撤出體外,內套管接負壓15~30 kPa持續吸引并間斷沖洗,外套管接500 mL生理鹽水。結腸外雙套管的外套管為普通橡皮引流管(內徑1.0 cm,長度30~40 cm),內套管為吸痰管(內徑0.7 cm、長度35~50 cm);結腸內雙套管的外套管為蕈狀導尿管,長度30~40 cm,內套管為吸痰管(內徑0.5 cm、長度35~50 cm)。雙套管內端均剪成魚口狀,距內管末端3~5 cm處剪3~4個側孔;內套管插入外套管中,距外套管外端約6 cm處穿出,雙套管兩管內端平齊,將內套管用絲線與外套管在穿出位置縫合固定。根據術中情況,外管在距內端約5~8 cm處剪數個橢圓形側孔。雙套管內、外套管側孔直徑分別約為1 mm和2 mm。

1.3 療效評價分別記錄3組的手術時間、術中出血量、排氣時間、住院時間、住院費用、術后感染情況、吻合口漏發生率等。術后對所有患者進行隨訪,隨訪時間為6~48個月,中位隨訪時間36個月。隨訪內容主要包括:患者生存狀態、生活品質、腫瘤復發情況等。
1.4 統計學方法采用SPSS 19.0進行統計學分析。計量資料以均數±標準差表示,多組間均數比較采用方差分析,組間多重比較采用SNK-q檢驗;計數資料組間比較采用χ2檢驗。以P<0.05為差異有統計學意義。
2.1 術后療效比較3組圍手術期均無病例死亡。3組間手術時間、術中出血量以及術后排氣時間差異無統計學意義(P>0.05)。引流組的住院時間和住院費用低于造口組(P<0.05),但與對照組相比差異無統計學意義(P>0.05),見表2。

2.2 術后并發癥及隨訪結果3組患者術后切口感染,盆、腹腔感染,肺部感染發生率及吻合口漏發生率差異均無統計學意義(P>0.05)。術后感染患者經積極對癥治療后均痊愈。經過加強全身營養支持,繼續無渣流質飲食,并保持腸腔內及吻合口旁的雙腔負壓引流(期間并未使用抗生素及生長抑素)后,造口組和引流組的吻合口漏患者均在4周內愈合并行還納術或拔出引流管,對照組27例吻合口漏患者中有6例在4周內愈合。隨訪結果表明,3組的復發、轉移及死亡比例差異均無統計學意義(P>0.05),見表3。

直腸癌是臨床上常見的消化道惡性腫瘤之一,其發病率有逐年上升的趨勢[6]。腸鏡技術和直腸癌根治手術的快速普及使得直腸癌能夠及早被發現并治療。在吻合器的輔助下,低位直腸癌患者實現了保肛治療。但Dixon術后吻合口漏的發生率居高不下,不僅增加患者痛苦,延長住院時間,部分患者需再次手術進行腸道轉流糞便才能治愈[7-8]。資料顯示按照TME原則手術治療直腸癌,吻合口漏的發生率為3%~19%[9]。目前認為吻合口張力過大引起的血運障礙是發生吻合口漏的主要因素,而盆、腹腔感染及引流不暢可加重血運障礙,影響吻合口的愈合[10]。針對吻合口漏發生的原因,臨床多采用如下措施:(1)加強圍手術期管理,營養支持,糾正貧血、低蛋白血癥,糖尿病患者控制血糖,術后早期予以腸內或靜脈營養,減輕腸道負荷。(2)術前腸道準備,徹底清潔腸道,減少手術污染。(3)手術時止血確切,充分游離腸管,行結直腸無張力吻合。(4)預防性末端回腸造口,防止小腸內容物過早進入結腸,使結腸處于空虛狀態,減少內容物對吻合口的刺激,促進吻合口的愈合。(5)充分引流,促進吻合口的愈合[11]。
為了降低術后吻合口漏的發生率,臨床部分采用了預防性的末端回腸造口術,將結直腸空置,促進吻合口的愈合[8,12]。但該方法需要行二期手術還納,住院時間長,費用高,增加患者痛苦[13]。本研究對比發現,傳統Dixon術(對照組)、Dixon術加預防性回腸末端造口術(造口組)和Dixon術加腸腔內、外雙套管負壓引流(引流組)3種手術方法治療直腸癌的療效相似,均未發生圍手術期死亡,且術后吻合口漏發生率、并發癥發生率及患者的復發、轉移和死亡比例無明顯差異。但與造口組相比,引流組無需行二次手術,縮短了住院時間,降低了住院費用,減輕了患者負擔。再者負壓引流可將術后腸腔內產生的氣體、消化液及術中殘留的糞渣及時排出體外,使腸腔始終保持清潔和空虛狀態。同時可充分引流腹腔滲出液,減少盆腔感染的發生,保持腸腔內空虛狀態有助于吻合口處腸管血運的改善及水腫消退。負壓主動吸引可使引流更有效,并可抵消術后因肛門收縮或結腸逆蠕動導致的腸腔內壓力增高[14],且雙套引流管的內套管可進氣進液,能防止組織吸附于引流管壁,既保證了引流的通暢,又避免了腸黏膜損傷。此外,一旦發生吻合口漏,腸腔內、外雙套管持續負壓引流能及時有效引流,防止漏出液擴散,促進吻合口漏的愈合[15]。由于樣本量有限,吻合口漏發生率總體不高,此次研究尚未得到回腸末端造口和雙套管負壓引流能夠顯著降低吻合口漏發生率的結論。為此,筆者將納入更多的直腸癌患者,以明確雙套管負壓引流對吻合口漏發生的防治作用。

Application of the double cavity casing negative pressure drainage by inside and outside of the intestine in the primary resection and anastomosis of low rectal cancer

ObjectiveTo investigate the feasibility and safety of the double cavity casing negative pressure drainage by inside and outside of the intestine in the primary resection and anastomosis of low rectal cancer.MethodsA total of 650 cases with low rectal cancer treated in our hospital from January 2009 to December 2014 were retrospectively collected and divided into control group(n=220),stoma group(n=205)and drainage group(n=225).The control group was received Dixon (low rectal anterior resection),the stoma group was treated with Dixon and ileostomy,while the drainage group was underwent double cavity casing negative pressure drainage by inside and outside of the intestine in the primary resection and anastomosis.The operation time,bleeding volume,the period of anal exhaust after operation and hospital expenses were compared between three groups.Postoperative follow-up was performed,and anastomotic leakage,postoperative infection and other complications were analyzed.ResultsThere were no significant differences in the operation time,bleeding volume and the period of anal exhaust after operation between three groups(P>0.05).The hospital stay and expenses were shorter and lower in drainage group than those in stoma group(P<0.05).In addition,there were no significant differences in wound infection,pelvic infection,and pulmonary infection between three groups(P>0.05).All patients were followed up, and the mortality,the recurrence rate and metastasis rate were not significantly different between three groups(P>0.05). ConclusionThe application of double cavity casing negative pressure drainage is likely to reduce the incidence of anastomotic leakage in Dixon,while the effect needs to be confirmed by large clinical trial,at the same time,patients enjoy shorter hospital stay,fewer suffering and lower expense of hospitalization.
