[摘要]細(xì)胞色素P450(CYP)2C8是人體CYP酶系的重要組成部分,并參與許多臨床常用藥物的代謝。目前,已報道的CYP2C8酶抑制劑數(shù)量不斷增加,針對中藥及其有效成分,中藥制劑和食物對CYP2C8的抑制作用研究也在逐步深入。通過查閱國內(nèi)外近幾年相關(guān)文獻(xiàn),進(jìn)行分析和歸納總結(jié),對食物、中藥及其有效成分對CYP2C8的抑制作用的體內(nèi)外研究進(jìn)展進(jìn)行綜述報道,重視中藥及其有效成分和食物與對CYP2C8底物藥物的代謝抑制所引起的藥物相互作用,以促進(jìn)藥物的合理使用,預(yù)防與中藥-西藥相互作用和食物-藥物相互作用相關(guān)藥物不良反應(yīng)的發(fā)生。

[Abstract] Cytochrome P4502C8 (CYP2C8) is an important part of the human CYP enzyme system (CYP450s) and is involved in the metabolism of many commonly used clinical drugs. The number of reported CYP2C8 enzyme inhibitors is increasing, and the research on the inhibitory effect of Chinese medicine preparations and food on CYP2C8 is also gradually deepened for Chinese medicine and its active ingredients. By analyzing and summarizing through referring to relevant literature in recent years at home and abroad, the research progress on the inhibition of CYP2C8 by food, Chinese herbal medicines, and their active ingredients both at home and abroad was reviewed. Attention should be paid to the study of drug-drug interaction caused by inhibition effects of Chinese herbal medicines, their active ingredients, and food in combination with other CYP2C8 substrates to promote the rational use of drugs and prevent the occurrence of adverse drug reactions related to Chinese medicine-Western medicine interaction and food-drug interaction.

細(xì)胞色素P450(CYP)2C8占人體CYP酶系(CYP 450s)總量的6%~7%,代謝至少1%經(jīng)Ⅰ相代謝的藥物[1]。約100種,其中按比例依次用于癌癥、糖尿病、呼吸系統(tǒng)、高脂血癥、高血壓及瘧疾等疾病的治療[2]。目前,對于中藥及其有效成分對CYP2C8的藥物相互作用(drug-drug interaction,DDI)研究目前報道尚不完善,中藥由于其成分復(fù)雜,在全世界均廣泛應(yīng)用,常與西藥合用的情況下,使得中藥對經(jīng)過CYP2C8代謝的藥物之間相互作用的研究對臨床中西藥合理搭配使用具有重要的指導(dǎo)作用。由于藥物治療疾病特殊性,為了保證其安全有效,藥物須按劑量正確服用。然而,食物的日攝入量因人而異,成分復(fù)雜,加之食物-藥物的相互作用很少引起臨床醫(yī)護(hù)人員重視,導(dǎo)致食物與藥物的相互作用發(fā)生隱匿且難以推算。因酶抑制作用是導(dǎo)致DDI和不良反應(yīng)發(fā)生的主要原因,本文總結(jié)了近幾年來國內(nèi)外有關(guān)食物、中藥材及其有效成分對CYP2C8的抑制作用的研究文獻(xiàn),以期為今后的科研、臨床用藥提供參考和借鑒。
1中藥材及其提取物對CYP2C8的抑制作用
中藥常以中藥飲片或提取物的形式入藥,中藥中的復(fù)雜成分對CYP2C8的綜合影響與單一活性成分對其的影響很可能有所不同。雖然其機(jī)制難以解釋,但是明確中藥材及其提取物與CYP2C8的關(guān)系,對臨床用藥更具實用性。有研究發(fā)現(xiàn),給大鼠單劑量或連續(xù)灌胃五味子或醋五味子提取物后,二者均可使紫杉醇的藥動學(xué)參數(shù)發(fā)生顯著變化,且醋五味子對CYP2C8的抑制作用強(qiáng)于五味子[3]。這提示了不同的炮制方法會影響中藥材對CYP2C8酶的代謝活性。
2中藥的有效成分單體對CYP2C8的抑制作用
對于有效成分明確的中藥,針對其主要活性成分的研究可以對其引起的臨床DDI的機(jī)制進(jìn)行合理的分析和推測。苦參提取物中黃酮類有效成分苦參酚Ⅰ、里查酮A和苦參黃酮G,三者在預(yù)溫敷和非預(yù)溫敷條件下均對CYP2C8表現(xiàn)為強(qiáng)烈抑制作用,研究者認(rèn)為需要進(jìn)行臨床實驗以便更好地預(yù)測苦參相關(guān)藥物對CYP2C8的影響[4]。Mukkavilli等[5]推測,生姜的有效成分若單獨與處方藥合用可能引起DDI,若以提取物的形式給藥,其抑制作用似乎與活性成分以外的其他成分有關(guān)。
Ab Rahman等[6]發(fā)現(xiàn)在體外人肝微粒體(human liver microsomes,HLM)中,姜黃素在10 μmol/L時可抑制91%的CYP2C8酶代謝活性。Bojic等[7]報道了白楊素(又名柯因)、柯因二甲醚和高良姜素對紫杉醇6-α-羥基化的抑制作用,三者均是CYP2C8的混合型抑制劑。紫檀芪廣泛存在于紫檀等中藥材中,其在體外抑制CYP2C8的作用較強(qiáng),日劑量指數(shù)(volumn per dose index,VDI)>50 L,表明其250 mg日劑量稀釋至50 L仍可達(dá)到足以抑制CYP2C8活性50%的濃度[8]。
此外,Park等[9]發(fā)現(xiàn)卷柏中四種雙黃酮均可在體外強(qiáng)烈抑制CYP2C8,Selamariscina A在CYP2C8酶體外研究中可被用作強(qiáng)陽性抑制劑。丹參的有效成分的體外半數(shù)抑制濃度(IC50)/可逆抑制常數(shù)值(Ki)遠(yuǎn)大于口服給藥后血藥濃度,因此丹參制劑口服給藥似乎不會引起DDI,但應(yīng)警惕丹參為主要成分的注射劑與CYP2C8底物合用的安全性[10]。
3中藥制劑對CYP2C8的抑制作用
關(guān)于中藥制劑對CYP2C8酶的研究較少,主要涉及臨床常用品種。復(fù)方苦參注射液在體外HLM中對CYP2C8存在較弱的抑制作用[11]。參照苦參提取物及其有效成分在體外實驗中對CYP2C8表現(xiàn)為強(qiáng)抑制作用,其結(jié)果與苦參注射液的體外抑制作用結(jié)果有很大差別。盡管體外實驗中,丹紅注射液和紅花注射液對CYP2C8酶抑制作用較強(qiáng)[12],但目前尚無體內(nèi)實驗報道,因此,通過CYP2C8代謝的化學(xué)藥物聯(lián)合使用這幾種中藥注射劑是否會引起DDI的體內(nèi)實驗尚需補(bǔ)充。李智等[13]研究了注射用益氣復(fù)脈(凍干)對HLM各亞型酶活性的影響,其中對CYP2C8的抑制作用IC50>6500 μg/ml,提示其引起DDI的可能性較低。近年來報道的中藥提取物、中藥有效成分單體及中藥制劑對CYP2C8的抑制作用研究結(jié)果見表1。
4食物對CYP2C8的抑制作用
Albassam等[25]發(fā)現(xiàn)蔓越莓提取物及鋸葉棕提取物的VDI值均>5,提示了引起DDI的可能性。此外,芒果干粉在體外對HLM CYP2C8也表現(xiàn)出中等程度的抑制作用,其IC50為37.93 μg/ml[26]。因此,臨床治療方案包含CYP2C8底物藥物時,也應(yīng)提倡相對固定食物攝入或者避免某些具有酶抑制作用食物的攝入。
5總結(jié)和展望
CYP2C8是肝臟中CYP450酶系中重要的代謝酶之一,近幾年針對CYP2C8酶抑制研究如火如荼,對中藥及其有效成分、中藥制劑和食物對CYP2C8酶代謝相互作用體外研究也陸續(xù)增多。但是目前該方面的研究存在一定的局限性,主要表現(xiàn)在:①中藥相關(guān)的對CYP2C8酶抑制體內(nèi)研究報道甚少。與化學(xué)藥物相比,中藥的成分復(fù)雜,因而其對CYP酶的影響也十分復(fù)雜,雖然體外實驗更加直觀和簡便,但是缺乏體內(nèi)環(huán)境的綜合影響,導(dǎo)致體內(nèi)外實驗結(jié)果常常不一致。②中藥的炮制方法、給藥劑量、給藥方案和給藥途徑等因素也會影響DDI的臨床意義,故應(yīng)結(jié)合體內(nèi)外實驗結(jié)果,從多方面、多角度來分析推測DDI發(fā)生的可能性。
綜上所述,研究中藥及其有效成分、中藥制劑和食物對CYP2C8的抑制作用,可以補(bǔ)充目前對CYP450酶系DDI研究的空白,為中西藥物的合理配伍提供指導(dǎo),同時為中藥及CYP2C8酶的相關(guān)研究提供思路和依據(jù)。
