摘要:目的 ?觀察頂蓋前區前核(AptN)是否參與心臟痛的調制。方法 ?選擇SD雄性大鼠18只,在戊巴比妥鈉(50 mg/kg) 腹腔麻醉下行心包插管術后,給予AptN分別施加強度10、25、50 μA的電刺激(波寬3 ms,頻率100 Hz),心包內注射辣椒素誘發心臟痛,記錄對側背斜方肌的肌電(EMG)。結果 ?電刺激APtN劑量依賴式抑制大鼠心包腔內注射辣椒素誘發的背斜方肌的EMG(Y=87.939-1.585X,r=0.951,P=0.049),EMG反應相對于基礎對照分別減小至(63.71±2.82)%、(37.65±5.75)% 和(15.71±4.30)%(P<0.05),肌電反應較對照明顯減弱,肌電幅值減小,放電頻率減少,放電時間縮短,潛伏期延長。結論 ?電刺激APtN,激活“APtN-中腦深部網狀結構、腦橋臂旁核及網狀旁巨細胞核-延髓外側到脊髓”的多突觸下行抑制通路,對心臟痛產生抑制性調節。

Abstract:Objective ?To observe whether AptN is involved in the modulation of cardiac pain.Methods ?18 SD male rats were selected. After pericardial intubation under intraperitoneal anesthesia with sodium pentobarbital (50 mg/kg), AptN was given to apply electrical stimulation of 10, 25, and 50 μA (wave width 3 ms,frequency 100 Hz), intrapericardial injection of capsaicin induced heart pain, and the electromyography (EMG) of the contralateral dorsi trapezius muscle was recorded.Results ?Electrical stimulation of APtN dose-dependently inhibited the EMG of rat dorsi trapezius induced by intrapericardial injection of capsaicin (Y=87.939-1.585X,r=0.951,P=0.049), and the EMG response was reduced to that of the basic control(63.71±2.82)%,(37.65±5.75)% and(15.71±4.30)%(P<0.05), the electromyographic response was significantly weaker than the control, the electromyographic amplitude decreased, the discharge frequency decreased, the discharge time shortened, and the latency period extend.Conclusion ?Electrical stimulation of APtN activates the multi-synaptic descending inhibitory pathways of "APtN-deep midbrain reticular structure, parapontine nucleus and parareticular giant cell nucleus-lateral medulla oblongata to spinal cord", which could inhibit cardiac pain.

心臟痛(cardiac pain)是心臟病患者發生心肌梗死時的典型癥狀之一,常警示患者的生命受到嚴重威脅,但是關于心臟痛覺信息的中樞調控作用迄今仍不明了,其作用機制更是所知甚少。來自內臟器官的疼痛信號不僅產生內臟痛,而且引起內臟-軀體運動反射[1]。研究表明[2,3],心包注射辣椒素可以產生一個可重復的心臟-軀體運動反射,該反射是研究心臟痛誘發痛覺過敏調制的適宜指標。頂蓋前區前核(the anteriorpretectal nucleus,AptN)是一個參與痛覺信息傳遞的中腦核團,并且參與下行痛覺調制[4,5],電刺激大鼠APtN可產生持久的鎮痛作用,但電刺激APtN是否能夠產生抑制心臟痛的作用,尚不清楚。本實驗用大鼠心臟-軀體運動反射模型,以心包內可復性注射辣椒素所誘發的背斜方肌的肌電(electromyogram,EMG)為指標,通過觀察背斜方肌EMG的變化,評估電刺激APtN對心臟傷害性感受的調節作用。
1材料與方法
