加味四逆散聯(lián)合針灸療法治療反流性食管炎的效果

摘要：目的 ?觀察加味四逆散聯(lián)合針灸療法在反流性食管炎治療中的應(yīng)用效果。方法 ?選取2018年9月～2019年9月在我院治療的74例反流性食管炎患者為研究對象，采用隨機數(shù)字表法分為對照組和觀察組，各37例。對照組采用常規(guī)治療，觀察組在對照組基礎(chǔ)上給予加味四逆散聯(lián)合針灸療法治療，比較兩組臨床治療總有效率、臨床癥狀（胃脘痛、噯氣反酸、飽腹感）消失時間、療程、治療前后膽汁反流（持續(xù)時間、頻率）情況、生活質(zhì)量及臨床不良反應(yīng)發(fā)生情況。結(jié)果 ?觀察組臨床治療總有效率為94.59%，高于對照組的81.08%（P<0.05）;觀察組胃脘痛、噯氣反酸、飽腹感消失時間、治療療程均短于對照組（P<0.05）;治療后兩組膽汁反流持續(xù)時間、頻率均低于治療前，且觀察組低于對照組（P<0.05）;治療后兩組生活質(zhì)量評分均高于治療前，且觀察組高于對照組（P<0.05）;觀察組不良反應(yīng)發(fā)生率為5.41%，低于對照組的16.21%（P<0.05）。結(jié)論 ?在常規(guī)西藥治療基礎(chǔ)上給予加味四逆散聯(lián)合針灸療法治療反流性食管炎有效率高，有助于進(jìn)一步改善臨床癥狀，縮短膽汁反流持續(xù)時間及頻率，改善患者生活質(zhì)量，值得應(yīng)用。

Abstract：Objective ?To observe the application effect of Jiawei Sini San combined with acupuncture in the treatment of reflux esophagitis.Methods ?A total of 74 patients with reflux esophagitis who were treated in our hospital from September 2018 to September 2019 were selected as the research objects. They were divided into a control group and an observation group using a random number table method， with 37 cases in each group. The control group was treated with conventional treatment， and the observation group was treated with Jiawei Sini San combined with acupuncture and moxibustion on the basis of the control group. The total effective rate of clinical treatment， clinical symptoms （gastric pain， belching acid reflux， satiety） disappeared time，the course of treatment， bile reflux （duration， frequency） before and after treatment， quality of life and occurrence of clinical adverse reactions.Results ?The total effective rate of clinical treatment in the observation group was 94.59%， which was higher than 81.08% in the control group （P<0.05）; the observation groups gastric pain， belching， acid reflux， satiety disappearance time and treatment course were shorter than those in the control group （P<0.05）; After treatment， the duration and frequency of bile reflux in the two groups were lower than before treatment， and the observation group was lower than that of the control group （P<0.05）; the quality of life scores of the two groups after treatment were higher than that before treatment， and the observation group was higher the control group （P<0.05）; the incidence of adverse reactions in the observation group was 5.41%， which was lower than 16.21% in the control group （P<0.05）.Conclusion ?On the basis of conventional western medicine treatment， the treatment of reflux esophagitis with Jiawei Sini San combined with acupuncture was effective， which helped to further improve the clinical symptoms， shortened the duration and frequency of bile reflux， and improved the quality of life of patients. It is worthy of application.

反流性食管炎（reflux esophagitis）是常見消化系統(tǒng)疾病，主要是由十二指腸、胃內(nèi)容物反流入食管引起的食管炎性病變，如果不及時有效治療可引起食管潰瘍，甚至是食管癌，嚴(yán)重威脅患者的生命安全。臨床常規(guī)給予胃動力、抑酸治療，可一定程度改善患者癥狀，但是不能徹底治愈，臨床效果欠佳。中醫(yī)學(xué)認(rèn)為反流性食管炎屬于“吞酸”“燒心”等范疇，其發(fā)生和肝氣不疏、飲食不節(jié)等相關(guān)。研究顯示[1]，以此為治療原則，采用標(biāo)本兼治的方法給予治療，同期輔以中醫(yī)針灸，可促進(jìn)藥效發(fā)揮，進(jìn)一步促進(jìn)臨床治療療效。加味四逆散治療反流性食管炎具有一定的療效，但是其對膽汁反流控制、患者生活質(zhì)量的影響尚未完全明確，有待大量的研究。本研究結(jié)合2018年9月～2019年9月在我院治療的74例反流性食管炎患者臨床資料，觀察加味四逆散聯(lián)合針灸療法在反流性食管炎中的應(yīng)用效果，現(xiàn)報道如下。……
