筋針聯合烏頭湯治療寒濕痹阻型早中期膝骨關節炎患者的臨床療效及其對血清CTX-Ⅱ、COMP的影響

Clinical efficacy of tendon acupuncture combined with Wutou Decoction in treatment of early and middle stage knee osteoarthritis of cold-dampness obstruction type and its effect on serum CTX-Ⅱ and COMP

[Abstract] Objective To observe the clinical efficacy of tendon acupuncture combined with Wutou Decoction in the treatment of early and middle stage knee osteoarthritis of cold-dampness obstruction type and its effect on serum CTX-Ⅱ and COMP. Methods A total of 80 patients with early to middle stage knee osteoarthritis of cold-dampness obstruction type treated in our hospital from June 2018 to June 2020 were randomLy divided into the treatment group and the control group， with 40 patients in each group.The treatment group were treated with tendon acupuncture combined with Wutou Decoction， and the control group were treated with Wutou Decoction. The changes of Western Ontario and McMaster University Osteoarthritis Index （WOMAC） scores， visual analog scale （VAS） scores， type Ⅱ collagen carboxy-terminal peptide （CTX-Ⅱ）， and oligomeric matrix protein （COMP） of the two groups before and after treatment were observed， to compare the clinical efficacy in the two groups. Results The total effective rate of the treatment group was 95.0%， which was higher than that of the control group （77.5%）， with statistically significant difference（P<0.05）. There were no statistically significant differences between the two groups in the scores of WOMAC and VAS before treatment（P>0.05）. After treatment， the WOMAC scores of the observation group were pain （6.20±2.12）points， stiffness （2.34±0.65）points and mobility function （23.56±7.68）points， and VAS score was （2.30±0.78）points， which were lower than those of the control group as （8.14±2.75） points， （3.56±1.06） points， （28.43±9.32） points， and （3.34±1.04） points， with statistically significant differences（P<0.05）. There were no statistically significant differences between the two groups in the levels of serum CTX-Ⅱ and COMP before treatment（P>0.05）. After treatment， the levels of serum CTX-Ⅱ and COMP of the observation group were （180.54±60.78） pg/mL and （467.37±155.25）pg/mL， which were lower than those of the control group as （201.91±70.19）pg/mL and （480.58±160.73） pg/mL， with statistically significant differences（P<0.05）. Conclusion Compared with single use of Wutou Decoction， tendon acupuncture combined with Wutou Decoction can better alleviate the clinical symptoms in patients with early and middle stage knee osteoarthritis of cold-dampness obstruction type， and improve the expression of serum CTX-Ⅱ and COMP.It has significant clinical efficacy.
