超聲對快速二期大動脈調轉術治療大動脈轉位的臨床研究

【摘要】 目的：探討超聲對快速二期大動脈調轉術治療完全性大動脈轉位（D—TGA）的臨床應用價值。方法：總結分析2005年8月—2011年6月15例施行快速二期大動脈調轉術的D—TGA患兒超聲心動圖表現，比較術前、術中、術后左心室的變化及手術結果，總結分析一期肺動脈環縮術后行二期手術的最佳時機。結果：15例患者一期術后，評估解剖左室，顯示室間隔凸向右室面，左心室呈圓形，提示實行二期手術時機已到。二期術后一個月內死亡4例，手術成功率約73.3%。術后3個月隨訪，1例死于重癥肺炎，其余10例主動脈、肺動脈吻合口血流通暢。結論：超聲心動圖對快速二期大動脈調轉術治療D—TGA的評估手術時機、術中監測以及術后隨訪均發揮著十分重要的作用。

【Abstract】 Objective：To evaluate the value of echocardiography in rapid two—stage arterial switch operation for patients with transposition of great arteries（D—TGA）.Method：15 patients of D—TGA underwent rapid two—stage arterial switch operation from August 2005 to June 2011.Trace above 15 cases during every phrases of surgery with echocardiography.Result：For 15 cases of rapid two—stage ASO，we could find a left to right bulging of the interventricular when the left ventricule had a circular shape and left ventricule diastolic dimension（LVDD） increased after pulmonary artery banding.Above reminded to two—stage AS0.4 patients died among 1 month after rapid two—stage arterial switch operation.3 months later after operation，1 patient died for lung inflammation.Conclusion：Echocardiography plays an important role in studying ASO during every phrases of surgery.

完全性大動脈轉位（D—transposition of the great arteries，D—TGA）是新生兒期最易發生心力衰竭、死亡率最高的紫紺型先心病。大動脈調轉術（arterial switch operation，ASO）治療D—TGA要求患兒在出生后3周內。若患兒診治時年齡稍大，左心室心肌退化，直接行ASO成功率較低[1]，可行快速二期大動脈調轉術。本研究總結分析施行快速二期大動脈調轉術的患兒超聲心動圖表現，探討超聲對該手術的術中檢測及術后療效評估的應用價值。

1 資料與方法

1.1 一般資料 2005年8月—2011年6月在鄭州大學第一附屬醫院和洛陽市中心醫院手術的共15例D—TGA患者，男9例，女6例；年齡52 d～2歲，中位年齡0.8歲；體質量4.7～15.0 kg。15例D—TGA中10例合并房間隔缺損，8例合并動脈導管未閉，10例合并小的室間隔缺損（3～6 mm）。15例患者均未合并肺動脈狹窄。……
