鼻內鏡下自體顳肌筋膜包裹篩骨垂直板修補鼻中隔穿孔療效觀察

目的 觀察鼻內鏡下應用自體顳肌筋膜包裹篩骨垂直板修補鼻中隔穿孔的手術療效。方法 回顧性分析50例鼻中隔穿孔患者的資料,其中傳統術式組30例,三明治式組20例。所有患者術前均行鼻內鏡及鼻竇CT檢查,并行鼻腔沖洗治療改善鼻腔黏膜狀況。傳統術式組行鼻中隔穿孔前切口,分離中隔黏膜移位修補穿孔;三明治式組行耳后顳肌附著處皮膚切口切取2倍以上穿孔直徑的顳肌筋膜,行鼻中隔前端切口,分離鼻中隔黏軟骨膜至穿孔四周,并在穿孔四周制作新鮮創面,以顳肌筋膜包裹自體篩骨垂直板形成硬性支撐“三明治”式修補物嵌插入黏軟骨膜下封閉鼻中隔穿孔;術后均予抗感染及鼻腔保濕治療。記錄術后癥狀恢復情況及隨訪結果。結果 三明治式組術中的出血量(mL:3.6±1.2 vs 13.4±2.9)、手術時間(min:25.2±6.3 vs 46.5±10.3)、損傷黏膜范圍(mm2:10.2±3.2 vs 20.5±5.3)及術后黏膜修復時間(d:19.5±3.9 vs 30.2±3.1)均小于傳統術式組(P<0.05);三明治式組的修補成功率高于傳統術式組(90.00%vs 46.67%,χ2=9.780,P<0.01);隨訪6個月~1年,均未見穿孔復發。結論 鼻內鏡下應用顳肌筋膜包裹自體篩骨垂直板,形成硬性支撐“三明治”式修補物封閉修補鼻中隔穿孔,可以恢復正常解剖結構的鼻中隔,療效肯定。

鼻中隔穿孔多由于手術、外傷、炎癥、腐蝕性化學物質、特殊感染等引起,可影響鼻腔通氣等生理功能[1]。手術修補是治療鼻中隔穿孔的唯一有效方法,傳統手術方式如減張縫合法、鼻中隔黏膜移位修補法等對大穿孔及中隔手術后穿孔的治療效果較差,成為鼻科醫生較為棘手的難題。近年來隨著鼻內鏡手術的廣泛開展,筆者所在科室在鼻內鏡下應用自體顳肌筋膜包裹篩骨垂直板形成硬性支撐“三明治”式修補物,封閉各種原因造成的不同大小的鼻中隔穿孔,取得了令人滿意的效果,報告如下。
1.1 一般資料 選取我院2008年1月—2013年12月收治的鼻中隔穿孔患者50例,男37例,女13例,年齡18~66歲,平均(35±1)歲,穿孔時間3個月~5年,平均2.5年。其中鼻中隔矯正術后致鼻中隔穿孔18例,鼻出血行微波燒灼止血致鼻中隔穿孔32例。主要癥狀為鼻腔干燥、鼻堵、頭痛、間斷鼻出血等。所有患者穿孔均位于鼻中隔軟骨前中部位,呈圓形或橢圓形,邊緣黏膜較薄、結痂,部分患者黏膜面上血管紋擴張。穿孔直徑0.6~0.9 cm者18例,>0.9~1.2 cm者32例。分為傳統術式組30例和三明治式組20例。
1.2 手術方法
1.2.1 術前準備 術前均行鼻內鏡及鼻竇CT掃描檢查以明確穿孔部位、直徑及篩骨垂直板殘留情況,每天以善邦沖洗劑(成都善邦生物科技有限公司)進行鼻腔沖洗,隨時清理鼻腔分泌物,鼻內鏡下清除穿孔緣周圍增生肉芽,應用薄荷油滴鼻,口服魚肝油、應用纖毛促排劑保證鼻腔黏膜清潔。
1.2.2 手術方法 (1)傳統術式。在鼻內鏡下兩側鼻腔黏膜以2%地卡因腎上腺素棉片收縮和表面麻醉,鼻中隔黏膜以1%利多卡因浸潤麻醉,在左側鼻腔鼻中隔穿孔前緣0.5 cm處做切口(穿孔位置靠前可行鼻小柱做切口),上至鼻頂,下達鼻底的弧形切口,切開黏-軟骨膜,緊貼殘存鼻中隔軟骨剝離鼻閾處皮膚及中隔黏膜,注意保護鼻中隔黏膜,剝離黏膜至穿孔四周,如遇局部黏連較緊,以小圓刀銳性剝離,切開穿孔邊緣黏膜,繼續向后剝離至篩骨垂直板,自左側鼻中隔黏膜自切口上端平鼻頂向后切開,轉向下方封閉鼻中隔穿孔處,確認完全封閉穿孔,雙側鼻腔填入高膨脹止血海綿固定鼻中隔黏膜。(2)三明治式。以1%利多卡因浸潤麻醉耳后顳肌附著處皮膚及皮下組織,首先做耳后切口約3 cm,剝離并暴露顳肌筋膜,以組織剪切取大小是穿孔直徑2倍以上的顳肌筋膜,展平晾干備用;然后在鼻內鏡下切口、剝離鼻中隔黏膜,方法同同傳統術式;繼續向后剝離至篩骨垂直板,切開四方軟骨剝離對側鼻中隔黏膜,在鼻內鏡下以黏膜剝離子環穿孔四周制造出新鮮黏膜創面;切取稍大于穿孔直徑的篩骨垂直板備用。將切取的篩骨垂直板置于兩層顳肌筋膜間以可吸收線將該三層組織貫穿縫合加以固定,形成三明治式修補物,將此夾帶骨組織的筋膜瓣作為封閉物,經鼻中隔切口植入穿孔處黏軟骨膜下,筋膜四周至少大于穿孔邊緣0.3 cm以上,調整兩側中隔黏膜平整覆蓋顳肌筋膜上。鼻中隔兩側放置膠原蛋白海綿,仔細貼附平整,滴入地塞米松液,使之呈膠凍狀,緊緊貼敷封閉物,再放入修剪合適的無菌塑料薄膜緊貼于穿孔中隔黏膜處,雙側鼻腔填入高膨脹止血海綿,手術結束。術后給予適量抗生素治療,72 h后抽出鼻腔填塞物。每天鼻腔應用重組表皮生長因子促進黏膜修復,鼻腔應用薄荷油點鼻,鼻朗海水噴鼻液(浙江朗柯生物工程有限公司)噴鼻,保持鼻腔濕潤,隨時清除創緣分泌物。
1.3 統計學方法 數據采用SPSS 15.0分析,數值變量采用±s表示,比較采用成組t檢驗,分類變量采用例(%)表示,比較采用χ2檢驗,以P<0.05為差異有統計學意義。
三明治式組術后7 d左右鼻中隔穿孔緣可見新生血管生長,見圖1,穿孔愈合時間3周,見圖2。三明治式組術中的出血量、手術時間、損傷黏膜范圍及術后黏膜修復時間均小于傳統術式組,見表1。三明治式組20例中18例手術均一期愈合,內鏡下示鼻中隔居中,黏膜光滑、完整,穿孔完全愈合。2例因既往有鼻中隔手術史,穿孔較大,篩骨垂直板殘留較少,術后穿孔縮小,經再次手術穿孔完全愈合;傳統術式組30例一期手術成功14例,成功率低于三明治式組(46.67%vs 90.00%,χ2=9.780,P<0.01)。術后隨訪6個月~1年,患者無任何鼻部不適癥狀,植入物未見排異反應及再穿孔現象。

常規的鼻中隔穿孔修補方法常選取穿孔周圍鼻中隔黏膜、穿孔水平的下鼻甲或中鼻甲黏膜、鼻底黏膜轉黏膜瓣進行修復,由于修補組織較少、穿孔水平、位置、鼻腔狹窄視野受限、特別是穿孔直徑>1 cm者造成操作困難,且修補后的單層組織血運較差、容易出現移位、壞死,由于游離鼻甲、鼻底、鼻中隔黏膜瓣[2-5],造成鼻腔正常解剖結構、生理功能的破壞等影響穿孔修補成功率。本文三明治式組采用自體篩骨垂直板作為硬性支撐,充分利用顳肌筋膜切取方便,不受穿孔大小、位置的局限,可多層切取、易成活,且避免了游離翻轉鼻腔正常結構的黏膜,避免了結構功能的破壞,切取足夠大的顳肌筋膜雙側包裹篩骨垂直板形成“三明治”式移植物。在穿孔兩側形成相對獨立的生長環境等同于將移植床的面積增加了1倍,明顯提高手術的成功率[6]。將移植物嵌插于穿孔的鼻中隔黏軟骨膜間,由于修補物大于穿孔直徑,將游離好的鼻中隔穿孔周圍的黏膜覆蓋顳肌筋膜,在穿孔處搭建一個有利于鼻中隔黏膜上皮生長的支架,增加了顳肌筋膜的血運,加快了顳肌筋膜的上皮化。顳肌筋膜較韌,易成活,可根據穿孔大小自由切取,自體篩骨垂直板無排異反應,支撐穩定,避免了筋膜出現皺褶、移動,恢復形成兩層黏膜夾一塊骨板的正常結構的鼻中隔,且很好地注意了黏膜的保潔、保濕,減少了手術損傷。本文中的20例三明治式組的手術時間、損傷黏膜范圍、手術中出血量,手術后愈合時間等均優于傳統手術方式組,修補后的鼻中隔更完整、黏膜光滑、平整,無論從形態還是功能上均接近正常生理解剖結構的鼻中隔。療效肯定,值得臨床推廣應用。

Effect of temporal fascia wrapping ethmoid perpendicular plate on repair of nasal septal perforation through endoscopic surgery

Objective To observe therapeutic effect of repairing nasal septum perforation with auto temporal fascia wrapping ethmoid perpendicular plate through endoscopic surgery.Methods Retrospective analysis of 50 cases of nasal septum perforation patient,among whom 30 cases were repaired by traditional surgery and 20 cases were repaired by sandwich surgery.All patients were examine by CT scan and nasal irrigation therapy were also performed through nasal endoscopy to improves nasal mucosa situation.Endoscopic procedures were performed in the traditional surgical group under localanesthesia,and septum mucosa was separated and shifted to repair septal perforation.On the other hand,temporal fascia of more than 2 times the diameter of the perforation were isolated then,wrapped around the perforation through front line of nasal incision after separating membrane of the nasal septum cartilage.Now,temporal fascia wrapping autologous ethmoid perpendicular plates form a rigid vertical"sandwich prosthesis"and were inserted into perichondrium of septal to repair perforation.Anti-infection and nasal moisturizing treatment were given after operation.Postoperative symptoms and the performance in follow-ups were recorded.Results Intraoperative blood loss(mL:3.6±1.2 vs 13.4±2.9),operative time(min:25.2± 6.3 vs 46.5±10.3),mucosal damage range(mm2:10.2±3.2 vs 20.5±5.3)and mucosal repair time postoperation(d:19.5±3.9 vs 30.2±3.1)were all less in sandwich group than those in traditional group(P<0.05).Successful rate in sandwich group is also higher than that in the traditional surgical group(90.00%vs 46.67%,χ2=9.780,P<0.01).No recurrence was found during follow-up from 6 months to 1 year in both groups.Conclusion Application of autologous temporalis fascia wrapping perpendicular plate of ethmoid bone to form a rigid"sandwich prosthesis"to repair nasal septal perforation by endoscopic surgery were effective.It can restore the normal anatomy of the nasal septum.
