雙切口內外側鋼板治療高能量脛骨平臺骨折65例臨床研究

[摘要] 目的 分析雙切口內外側鋼板治療復雜性脛骨平臺骨折臨床療效。方法 方便選取從2011年6月—2015年10月該院收治的65例復雜性脛骨平臺骨折，其中35例Schazker V型，30例Schazker VI型，采取雙切口內外側鋼板內固定治療，分析膝關節恢復情況以及脛骨平臺內翻角、后傾角。結果 骨折愈合時間平均為4個月，未出現關節面塌陷或者是內外翻畸形，優良率為81.53%。術后即刻、術后6個月脛骨平臺內翻角及后傾角差異無統計學意義（P>0.05）。 結論 對復雜性脛骨平臺骨折采用雙切口內外側鋼板治療，效果顯著，具有臨床應用價值。

Clinical Study on 65 Cases of High Energy Tibial Plateau Fractures Treated with Double Incision and Internal Lateral Plate

[Abstract] Objective To analysis two incision and lateral plate for the treatment of complex tibial plateau fractures in clinical efficacy. Methods Convenient selection 65 cases of complex tibial plateau fracture from June 2011 to October 2015 including 35 cases of Schazker type V， 30 cases of Schazker type VI， adopt double incision and lateral plate fixation， analysis of knee joint recovery and tibial plateau angle， angle. Results The fracture healing time averaged 4 months. There was no articular surface collapse or varus deformity， the excellent and good rate was 81.53%. Postoperative instantly， after six months of tibial plateau varus angle and angle without significant difference（P>0.05）.Conclusion The treatment of complex tibial plateau fracture with double incision and lateral plate， the effect is significant， and it has the value of clinical application.

臨床中，復雜性脛骨平臺骨折是指雙踝骨折（Schazker V型）、雙踝骨折伴干骺端骨折（Schazker VI型），會累及干骺端、脛骨干、及脛骨雙側平臺，通常伴血管、韌帶、關節內半月板等損傷。復雜性脛骨平臺骨折主要是由高能量創傷所導致的，易引起軟組織損傷、塌陷以及內外側平臺壓縮等，并發癥多，增加手術難度[1-3]。不及時采取治療，會對膝關節生理功能產生影響，所以需選取合理手術方法。該文主要分析雙切口內外側鋼板治療，從2011年6月—2015年10月該院收治的65例復雜性脛骨平臺骨折效果，現報道如下。

1 資料與方法

1.1 一般資料

方便選取該院收治的65例復雜性脛骨平臺骨折，該組患者術前均采取小腿正側位、膝關節X線，膝關節采取CT掃描與三維重建。其中33例女，32例男，年齡為20～70歲，平均為（37.56±7.44）歲；Schazker分型：35例V型，30例VI型；58例閉合骨折，7例開放骨折；……
