導樂陪伴分娩在初產婦陰道分娩中的臨床應用

摘要：目的 探討導樂陪伴分娩在初產婦分娩過程中對剖宮產率，無指征剖宮產率、產后出血、新生兒窒息率和患者滿意度及糾紛發生的影響。方法 回顧性分析我院2012年～2015年400例陰道分娩的初產婦，將200例實行了導樂陪伴分娩的初產婦作為研究組，200例實行非導樂陪伴分娩的初產婦作為對照組，進行臨床對比觀察。結果 導樂陪伴分娩組剖宮產率下降（P<0.001），無指征剖宮國產下降（P<0.05），產后出血率下降（P<0.05），新生兒輕度窒息率降低（P<0.05），患者滿意率增加（P<0.01），減少產科抱怨或投訴（P<0.05）。結論 導樂陪伴分娩能增強初產婦陰道分娩的信心，既能降低剖宮產率，尤其是減少個人要求的剖宮產又能減少產后出血，還能保護產婦和新生兒安全，能增進醫護和產婦的溝通，提高產婦及家屬滿意度，減少醫患糾紛，提高產科質量。

Analysis of the Effect of Doula Services for Laboring Women in Early Maternal Vaginal Delivery

Abstract：Objective To explore the effects of doula services for laboring women on the incidence of cesarean section， cesarean section without indications， postpartum hemorrhage and neonatal asphyxia， and occurrence of patient satisfaction and disputes. Methods 200 primipara with normal delivery as control group and 200 with doula services for laboring womenas research group in the Maternal and Child Health Hospital of Yichang city during 2012 and 2015 were selected， and we compared the delivery outcomes between the two groups. Results The incidences of cesarean section， cesarean section without indications， postpartum hemorrhage and neonatal asphyxia were lower in the research group than those in the control group， and the satisfaction rate among the patients was higher in the research group with less complaints. Conclusion Doula services for laboring womencan enhance the confidence of pregnant women for vaginal delivery， thus reducing the incidences of cesarean section， cesarean section without indications， postpartum hemorrhage and neonatal asphyxia. In the meanwhile， it can promote doctor-patient communication， reduce doctor-patient disputes， and improve the clinical quality of obstetrics.

導樂陪伴分娩是在Doula式分娩基礎上，以產婦為中心的護理模式，通過有效的護理管理，提供高質量、適合母嬰需要的適宜技術，以減少醫源性產科干預，降低剖宮產率、提高自然分娩率，達到母嬰安全的產時服務新模式。為了促進自然分娩，保障母嬰安康，我院于2010年開展導樂分娩，并逐漸完善流程，總結經驗，取得了滿意效果，具體報道如下。
