貧困地區(qū)公立醫(yī)院住院費(fèi)用構(gòu)成及成本控制指標(biāo)分析評(píng)價(jià)

摘要：收集某貧困地區(qū)一所市級(jí)三甲綜合醫(yī)院2014年60435人次的住院患者的病案資料，對(duì)患者在住院期間不同時(shí)段費(fèi)用構(gòu)成及影響因素進(jìn)行統(tǒng)計(jì)分析，探詢各科室患者住院期間各階段發(fā)生費(fèi)用原因，這對(duì)于貧困地區(qū)公立醫(yī)院有效控制成本，合理地調(diào)整住院患者費(fèi)用結(jié)構(gòu)，進(jìn)一步制訂控制衛(wèi)生費(fèi)用增長(zhǎng)的措施提供決策依據(jù)。

Analysis and Evaluation of the Cost Structure and Cost Control Indexes of Public Hospitals in Poor Areas

Abstract：60435 medical records information of hospitalized patients from a three level first class general hospital in a poverty area in 2014 were collected to analysis constitution of hospital costs of patients and influencing factor during different period of hospitalization. Digging expense cause of patients in different department during different period of hospitalization to provide decision basis for effectively monitor cost， reasonable adjust constitution of hospital costs and formulate control of health costs.

國(guó)務(wù)院辦公廳關(guān)于城市公立醫(yī)院綜合改革試點(diǎn)的指導(dǎo)意見(jiàn)國(guó)辦發(fā)〔2015〕38號(hào)的精神要求，基本目標(biāo)總體上個(gè)人衛(wèi)生支出占衛(wèi)生總費(fèi)用的比例降低到30%以下[1]。基本路徑建立公立醫(yī)院科學(xué)補(bǔ)償機(jī)制，以破除以藥補(bǔ)醫(yī)機(jī)制為關(guān)鍵環(huán)節(jié)，建立符合醫(yī)療行業(yè)特點(diǎn)的薪酬制度[1]。該院患者住院總費(fèi)用逐年呈15%以上遞增，筆者對(duì)市級(jí)三級(jí)綜合醫(yī)院患者在住院期間各科室、各階段住院費(fèi)用分析如下。

1 資料與方法

1.1一般資料 貧困地區(qū)某市級(jí)醫(yī)院2014年1月～12月出院患者61961人次，由于1033人次病歷首頁(yè)填寫(xiě)不全，實(shí)際有效統(tǒng)計(jì)60435人次的病例，從醫(yī)院病案室信息庫(kù)選取的病例作為研究對(duì)象。

1.2方法 疾病分類采用國(guó)際疾病分類（ICD-10），采用SPSS19.0統(tǒng)計(jì)軟件包進(jìn)行統(tǒng)計(jì)學(xué)處理，分析各項(xiàng)住院費(fèi)用的基本特征及費(fèi)用構(gòu)成，以P<0.05為有統(tǒng)計(jì)學(xué)意義。

2 結(jié)果與分析

2014年全年住院患者平均住院費(fèi)用8783.41元，住院期間前3 d平均住院費(fèi)用3536.25元，占全年住院患者平均住院費(fèi)用的40.26%；住院期間前4～7 d平均住院費(fèi)用1801.72元，占全年住院患者平均住院費(fèi)用的20.51%；住院期間8 d至出院平均住院費(fèi)用3445.47元，占全年住院患者平均住院費(fèi)用的39.23%。……
