骨科損傷控制在不穩定骨盆骨折合并四肢多發骨折治療中的應用效果觀察

摘要：目的 研究骨科損傷控制在不穩定骨盆骨折合并四肢多發骨折治療中的應用效果。方法 將我院2015年1月～6月不穩定骨盆骨折合并四肢多發骨折患者56例分兩組，常規組行常規處理；損傷控制組應用骨科損傷控制。就兩組患者手術過程出血情況、手術后住院平均時間、術后骨折愈合時間和不穩定骨盆骨折合并四肢多發骨折X線片結果優良率、尿道損傷、感染、靜脈血栓等并發癥發生率進行比較。結果 損傷控制組不穩定骨盆骨折合并四肢多發骨折X線片結果優良率明顯高于常規組，尿道損傷、感染、靜脈血栓等并發癥發生率明顯低于常規組，P<0.05。損傷控制組患者手術過程出血情況、手術后住院平均時間、術后骨折愈合時間均明顯優于常規組，P<0.05。結論 骨科損傷控制在不穩定骨盆骨折合并四肢多發骨折治療中的應用效果確切，可有效控制創傷，減少出血，減少術后并發癥的出現，有助于加速手術后骨折愈合，縮短術后住院時間，是一種有效的處理方法，值得推廣。

Abstract：Objective To study the effect of orthopedic injury control in the treatment of unstable pelvic fractures combined with multiple fractures of the extremities.Methods 56 patients with unstable pelvic fractures and multiple extremities fractures in our hospital from January to June 2015 were divided into two groups.The conventional group received routine treatment.Damage control group application of department of orthopedics injury.Two groups patients of bleeding，the average time of hospitalization after operation，fracture healing time and unstable pelvic fracture combined with multiple fractures of limbs radiography showed excellent rate of postoperative urethral injury，infection，venous thrombosis and other complications were compared.Results The injury control group unstable pelvic fracture combined with multiple fractures of limbs radiography showed excellent rate was significantly higher than that of conventional group，urethral injury，infection，venous thrombosis and other complications were significantly lower than the conventional group，P<0.05.Damage control group of patients with bleeding，the average time of hospitalization after operation，fracture healing time were significantly better than the conventional group，P<0.05.Conclusion The application of orthopedic injury control in treatment of unstable pelvic fractures combined with multiple fractures of extremities is effective，which can effectively control the trauma，reduce the bleeding，reduce the occurrence of postoperative complications，accelerate the healing of fractures after surgery，and shorten the postoperative hospital stay，is an effective approach that is worth promoting.

不穩定骨盆骨折在臨床上發生率高，屬于高發骨折類型，患者常可合并四肢多發骨折、臟器損傷等，癥狀嚴重，病情復雜，具有較高的致殘率和死亡率，若處理不及時可導致患者死亡。骨科損傷控制理論在骨科損傷中應用廣泛，其是根據患者耐受情況和創傷情況，采取分期治療的手段實現對患者的有效治療，可有效控制損傷，預防大出血等不良事件的發生，維持患者生命體征穩定，并在良好的生命體征狀態下進行手術治療[1]。本研究探討了骨科損傷控制在不穩定骨盆骨折合并四肢多發骨折治療中的應用效果，報道如下。endprint
