護理不良事件成因分析及防范對策

摘要：目的 對護理不良事件進行總結分析找出防范對策，降低不良事件發生率，保障護理安全。方法 通過對本院2015年1月～2016年6月發生的128例護理不良事件類型、發生時間、環境、人員等因素進行統計分析，找出原因制定防范對策。結果 在128例不良事件中依次以預防不足、輸液問題、用藥錯誤為主，時間以白班居多，夜間以深夜2：00～8：30為首；人員以低年資護士居多；其次與護理人員評估不足，患者及家屬依從性差等因素相關。結論 通過科學的管理機制，制定針對性防范對策，可減少不良事件發生率，保障護理安全。

Abstract：Objective To summarize and analyze the adverse events of nursing to find the preventive measures，reduce the incidence of adverse events and ensure the safety of nursing.Methods The hospital from January 2015 to June 2016 occurred in 128 cases of adverse event types，occurrence time，environment，personnel and other factors were statistically analyzed to find out the reasons to develop preventive measures.Results Among 128 adverse events，there were insufficient preventive measures， infusion problems and medication errors.Most of the time was on a day shift.Night 2：00～8：30 at night led to the majority of staff with low seniority nurses；followed by inadequate assessment of nursing staff，patients and their dependents and poor family members.Conclusion Through scientific management mechanism，we can make targeted prevention measures to reduce the incidence of adverse events and ensure the safety of nursing.

護理不良事件目前尚無統一的定義，護理不良事件是指在護理過程中發生的、 不在計劃中的、未預計到的或通常不希望發生的事件，包括患者在住院期間發生的跌倒、用藥錯誤、走失、誤吸或窒息、燙傷及其他與患者安全相關的、非正常的護理意外事件[1]。護理不良事件的發生常會危及病人的健康和生命安全，給患者及家庭造成不幸，同時會使醫療機構在人、財、物、聲譽、形象方面受到損失[2]，本文就128例護理不良事件進行回顧性分析，制定防范措施。

1資料與方法

1.1一般資料

我院為一所三級綜合醫院，收集本院2015年1月～2016年6月128例護理不良事件。本院不良事件上報管理均為非懲罰式，發生不良事件后當事人填寫不良事件上報表及相關因素分析表報護理部。相關因素分析表涉及五個維度包括管理、人員、環境、設備、材料?！?br>
