基于VAS評(píng)分改進(jìn)的護(hù)理模式對(duì)糖尿病足患者的影響

摘? ?要：目的? 探討視覺(jué)模擬評(píng)分在糖尿病足患者護(hù)理中的應(yīng)用效果。方法? 選取2016年1月～2017年3月對(duì)84例糖尿病足患者進(jìn)行常規(guī)護(hù)理的同時(shí)采用VAS量表進(jìn)行疼痛評(píng)分，根據(jù)評(píng)分結(jié)果給予及時(shí)有效的護(hù)理，觀察患者護(hù)理干預(yù)前后疼痛程度、血糖達(dá)標(biāo)率以及滿意度情況。結(jié)果? 干預(yù)后患者疼痛癥狀1～3分者占64.29%，4～6分者占29.76%，7～9分者占5.95%，無(wú)疼痛癥狀10分者，疼痛程度較干預(yù)前減輕，差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。干預(yù)后血糖達(dá)標(biāo)率和滿意度分別為85.71%和94.27%，均高于干預(yù)前的8.33%和65.12%，差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論? 采用VAS評(píng)分改進(jìn)的護(hù)理模式，可以有效改善糖尿病足患者的疼痛癥狀，控制血糖水平，提高其生活質(zhì)量。

Effect of Improved Nursing Model Based on VAS Score on Patients with Diabetic Foot

Abstract：Objective? To investigate the effect of visual analogue scale on the nursing of patients with diabetic foot.Methods? From January 2016 to March 2017，84 patients with diabetic foot were treated with routine care，and the VAS scale was used for pain score.According to the score results，timely and effective nursing was given，and the degree of pain，blood glucose compliance rate and satisfaction were observed before and after the patient's nursing intervention.Results? After intervention，patients with pain symptoms 1 to 3 points accounted for 64.29%，4 to 6 points accounted for 29.76%，7 to 9 points accounted for 5.95%，and those without pain symptoms scored 10 points.The degree of pain was less than before intervention，the difference was statistically significant（P<0.05）.The compliance rate and satisfaction of blood glucose after intervention were 85.71% and 94.27%，respectively，which were higher than 8.33% and 65.12% before intervention，the difference was statistically significant（P<0.05）.Conclusion? The improved nursing mode with VAS score can effectively improve the pain symptoms of diabetic foot patients，control blood sugar levels and improve their quality of life.

糖尿病足（diabetic foot，DF）是糖尿病的一種常見(jiàn)并發(fā)癥，是糖尿病患者致殘的重要原因，糖尿病患者大約有12%～25%會(huì)罹患糖尿病足病[1]。足部疼痛是糖尿病足病的重要表現(xiàn)，臨床上糖尿病足患者疼痛的治療經(jīng)常會(huì)被忽視。研究表明[2]，對(duì)糖尿病足潰瘍伴有疼痛的患者，其疼痛癥狀不僅不利于傷口的愈合，也嚴(yán)重影響了該類患者的生活質(zhì)量，患者極易出現(xiàn)睡眠不佳、情緒不穩(wěn)等現(xiàn)象，給護(hù)理工作帶來(lái)一定的困難。另外，疼痛也會(huì)影響患者血糖的控制，而患者血糖控制不佳同樣會(huì)影響糖尿病足的恢復(fù)。所以，糖尿病足患者的疼痛護(hù)理顯得尤為關(guān)鍵。我科采用基于視覺(jué)模擬評(píng)分（VAS）改進(jìn)的護(hù)理模式對(duì)糖尿病足患者進(jìn)行護(hù)理分析，現(xiàn)報(bào)告如下。

1資料與方法

1.1一般資料? 選取2016年1月～2017年3月萍鄉(xiāng)市人民醫(yī)院內(nèi)分泌科收治的糖尿病足患者84例，本研究經(jīng)醫(yī)院倫理會(huì)批準(zhǔn)。納入標(biāo)準(zhǔn)：①符合世界衛(wèi)生組織糖尿病診斷標(biāo)準(zhǔn)，糖尿病足的診斷標(biāo)準(zhǔn)符合第一屆全國(guó)糖尿病足學(xué)會(huì)制定的檢查方法和診斷標(biāo)準(zhǔn);②年齡≤80歲;③足部無(wú)外傷者;④能配合檢查和治療者;⑤簽訂知情同意者。……
