[摘要]目的:探討濃縮生長因子纖維蛋白(Concentrated growth factor fibrin,CGF)治療牙齦退縮的效果及對牙齦成纖維細胞(Human gingival fibroblast,HGF)增殖的影響。方法:選擇2018年6月-2019年6月筆者醫院收治的30例牙齦退縮患者為研究對象,采集患者靜脈血制備CGF膜,將CGF膜固定于術區進行治療,觀察術前及術后6個月的牙周袋深度(Pocket depth,PD)、退縮高度(Heigh of recession,HR)、角化齦寬度(Keratinized mucosa width,KMW)、紅色美學評分(Pink aesthetic score,PES)和白色美學評分(White aesthetics score,WES)。取正常牙齦組織,采用貼壁法傳代培養成HGF,在電鏡下觀察CGF膜上HGF的黏附情況,采用CCK-8法檢測HGF的增殖情況。結果:與術前比較,術后6個月患者的PD[(2.89±0.57)mm vs (2.58±0.36)mm]、KMW[(3.99±0.68)mm vs (3.55±0.64)mm]、PES[(7.42±1.63)分 vs (2.80±0.59)分]、WES[(7.90±1.74)分 vs (2.77±0.55)分]均明顯增高(P<0.05),HR[(1.95±0.56)mm vs (2.97±0.48)mm]明顯降低(P<0.05)。通過電鏡能夠清晰看到HGF呈層層黏附于CGF膜上;HGF的增殖檢測結果顯示,培養1d、4d、7d時,CGF組的OD值[(0.54±0.08) vs (0.34±0.07)]、[(0.85±0.12) vs (0.55±0.14)]、[(1.13±0.30) vs (0.66±0.18)]均明顯高于對照組(P<0.05)。結論:CGF治療牙齦退縮的臨床效果及美學效果均顯著,能夠明顯促進HGF的黏附、生長和增殖。

Curative Effect of Concentrated Growth Factor Fibrin in the Treatment of Gingival Recession and its Impact on Proliferation of Human Gingival Fibroblasts

Abstract:Objective? To discuss the curative effect of concentrated growth factor fibrin(CGF) in the treatment of gingival recession and its impact on proliferation of human gingival fibroblasts(HGF). Methods? 30 patients with gingival recession who was taken as research objects in our hospital from June 2018 to June 2019. Venous blood was collected to prepare CGF membrane, which was fixed in the operative area for treatment. The pocket depth(PD), heigh of recession(HR), keratinized mucosa width(KMW), pink aesthetic score(PES),white aesthetics score(WES) was observed before operation and 6 months after operation. The adhesion of HGF on CGF membrane was observed under electron microscopy. The proliferation of HGF was detected by CCK-8 method. Results Compared to before operation, the PD[(2.89±0.57)mm vs (2.58±0.36)mm],KMW[(3.99±0.68)mm vs (3.55±0.64)mm], PES[(7.42±1.63)scores vs (2.80±0.59)scores], WES[(7.90±1.74)scores vs (2.77±0.55)scores] of patients were significantly increased and the HR[(1.95±0.56)mm vs (2.97±0.48)mm] of patients were significantly decreased 6 months after operation(P<0.05). By means of electron microscopy, it was clear that HGF adhered to CGF membranelayer by layer. The results of proliferation detection of HGF showed, cultured for 1d, 4d and 7d, the OD value[(0.54±0.08) vs (0.34±0.07)], [(0.85±0.12) vs (0.55±0.14)], [(1.13±0.30) vs (0.66±0.18)] of CGFgroup was significantly higher than that of control group(P<0.05). Conclusion The clinical and aesthetic effects were significant of CGF in treatment of gingival recession. CGF could significantly promote the adhesion, growth, proliferation of HGF.
膜齦術式繁多,其最終目的都是為了獲取完全的根面覆蓋和良好的美觀效果,國內外學者將不同術式聯合不同的生物材料,輔助于促進牙齦軟組織的愈合[10]。人牙齦成纖維細胞是人牙齦結蹄組織中的主要細胞,主要存在于牙齦固有層中,其主要功能是合成膠原纖維、彈性纖維及基質,具有較活躍的自身更新能力,在牙周軟組織的自身修復及創傷愈合中起著至關重要的作用[15]。有研究[16]分析兩種富血小板纖維蛋白產物對于牙齦成纖維細胞的影響,結果表明富血小板纖維蛋白膜對于HGF增殖有促進作用。本研究還通過體外實驗探討了CGF對HGF增殖的影響。結果顯示,通過電鏡能夠清晰看到HGF呈層層黏附于CGF膜上,HGF的增殖檢測中,培養1d、4d、7d時,CGF組的OD值均明顯高于對照組。說明CGF體外促進HGF的效果較好,并證實了CGF在促進HGF的增殖中起重要作用。
綜上所述,CGF具有來源豐富、高效安全,無排斥反應等優點。CGF治療牙齦退縮的臨床效果及美學效果均顯著,能夠明顯促進HGF的黏附、生長及增殖,這一結果對臨床應用CGF輔助于牙齦促進軟組織再生提供了重要的理論依據。
