根管治療術后磨牙行高嵌體或全冠對患牙的保護作用比較

[摘要] 目的 比較根管治療術后磨牙行高嵌體或者全冠對患牙的保護作用。 方法 將2016年1月～2018年1月間我院接收初次開展根管治療且僅有一個患牙的磨牙患者共60例進行研究，根據數字表法隨機分成兩組，每組30例（30顆患牙），對照組開展全冠修復，觀察組開展高嵌體修復，兩組治療后1周與1年分別進行隨訪，觀察兩組的牙周組織健康、美觀及牙齒保存成功情況。 結果 治療后1周，兩組在牙周組織健康和美觀方面差異不顯著（P>0.05）。治療后1年，兩組在美觀和牙齒保存成功方面差異不顯著（P>0.05）;觀察組在牙周組織健康方面比對照組更優（P<0.05）。 結論 在根管治療后對磨牙開展高嵌體修復是有效安全的，能對牙體組織起到保護作用，給牙周組織帶來的刺激更小。

Comparison of protective effects of onlays and full crowns on affected molars after root canal treatment

[Abstract] Objective To compare the protective effects of onlays and full crowns on the affected molars after root canal treatment. Methods A total of 60 patients with only one affected molar who was given the initial root canal treatment in our hospital from January 2016 to January 2018 were enrolled and divided into two groups according to the number table method， with 30 patients （30 affected teeth） in each group. Full crown repair was performed in the control group and onlay repair was performed in the observation group. The two groups were followed up 1 week and 1 year after treatment when the periodontal tissue health， aesthetics and tooth preservation success of the two groups were observed. Results One week after treatment， there were no significant differences in periodontal tissue health and aesthetics between the two groups（P>0.05）. One year after treatment， there were no significant differences in aesthetics and tooth preservation success between the two groups （P>0.05）. The observation group was superior to the control group in terms of periodontal tissue health （P<0.05）. Conclusion Implementation of onlay repair on the molar is effective and safe after root canal treatment， which can protect the tooth tissue and bring less stimulation to the periodontal tissue.

根管治療屬于目前保存磨牙的常用方法，其將發炎牙髓摘除，經根管消毒，開展嚴密充填以達到治療效果，能使患齒不再腫痛與發炎，繼續發揮其磨碎、切斷食物等應有的作用，但牙體經過根管治療后，抗折能力會有一定下降，脆性明顯增加，極易出現折斷，使用時極有可能出現劈裂等危險事故，因此需要積極采取相應的保護措施，以增強牙根抗折性能[1-2]。以往選擇牙本質釘、玻璃離子、樹脂以及銀汞等進行直接修復，極易導致充填物脫落、患牙折裂等[3]。怎樣做好患牙的保護工作，盡可能使其壽命延長已逐漸成為牙科面臨的一項嚴峻挑戰。近幾年來，高嵌體開始被廣泛應用于臨床，使得修復成功率有了明顯提高[4]。還有研究表明[5]，開展冠修復需進行大量牙體預備，同時會給健康牙體組織帶來較大損傷，使牙體抗折能力進一步降低。本文選取……
