Application of comprehensive decongestive lymphatic therapy in patients with upper limb lymphedema after breast cancer surgery

[Abstract] Objective To study the effect and application of comprehensive decongestive lymphatic therapy in the clinical treatment of upper limb lymphedema after breast cancer surgery. Methods A total of 32 patients with lymphedema of upper limbs after breast cancer surgery in our hospital from September 2017 to September 2019 were selected.All patients were treated with comprehensive decongestive lymphatic therapy.The clinical effect before and after treatment was observed and compared，including the patient's tissue water ratio，circumference ratio，circumference above and below the elbow stripes，underarm circumference，the circumference of tiger's mouth and above the wrist stripes， Bathel index and Fugl-Meyer score. Results Compared with before treatment，the tissue water ratio and circumference ratio of patients with upper limb lymphedema was shorter after treatment.The application effect of comprehensive decongestive lymphatic therapy was better （P<0.05）. After treatment，patients with upper limb lymphedema after breast cancer surgery had shorter circumferences above the elbow stripes，below the elbow stripes，underarms，tiger's mouth，and upper wrist stripes （P<0.05）. Compared with before treatment， the Bathel index and Fugl-Meyer score of patients with upper limb lymphedema after breast cancer surgery were higher （P<0.05）. Conclusion The comprehensive decongestive lymphatic therapy has an ideal application effect.It is of great significance for improving the clinical symptoms of patients and relieving lymphedema of the upper limbs.It has a definite impact and is worth popularizing in clinical treatment.

乳腺癌是我國臨床中，屬于多發的一種女性腫瘤疾病，我國每年新發乳腺癌約26.9萬例，死亡約7.0萬例，40～60歲女性為高發年齡段人群，且愈加年輕化，該疾病是發生在乳腺腺上皮組織的惡性腫瘤疾病[1]。腋窩淋巴結清掃治療是我國現階段治療乳腺癌的標準方式之一，但術后少數患者容易發生上肢淋巴水腫情況，這將對患者的疾病恢復造成不良影響[2]。近年來綜合淋巴消腫療法廣泛應用于臨床相關疾病治療中，其對于乳腺癌術后上肢淋巴水腫患者具有良好的臨床療效[3]。根據相關報告指出，將綜合淋巴消腫療法應用于乳腺癌術后上肢淋巴水腫疾病的臨床治療中，療效確切。同時有利于促進滯留組織淋巴水腫回流，改善肢體腫脹情況。但此方面報告少之又少，仍缺乏相關臨床依據。在本次實驗中，對研究組和對照組32例乳腺癌術后上肢淋巴水腫患者的治療效果展開討論和分析，其目的在于研究綜合淋巴消腫方案對于乳腺癌術后上肢淋巴水腫疾病的臨床療效，現報道如下。
