多層螺旋CT低劑量掃描技術在83例兒童急性闌尾炎中的診斷價值分析

The diagnostic value analysis of low-dose multi-slice helical CT scan in 83 children with acute appendicitis

[Abstract] Objective To explore the diagnostic value of low-dose multi-slice helical CT scan in children with acute appendicitis. Methods A total of 165 patients admitted to our hospital from December 2016 to December 2020 were randomly divided into observation group（n=83） and control group（n=82）. The observation group used Brilliance nano low-dose 128-slice helical CT scan， and the control group used Brilliance nano conventional-dose 128-slice helical CT scan.The scan dose， effective milliampere second， the positive detection rate between the two groups were observed and compared. Results The scan dose of the observation group was （8.29±2.38）mGy，? lower than that of the control group （19.33±4.51）mGy. The effective milliampere second （86.73±23.08）mAs in the observation group was lower than that （162.02±47.71） mAs in the control group. The difference between the two groups was statistically significant（P<0.05）. The positive detection rate of the observation group was 92.77% （77/83）， and the positive detection rate of pathological results was 96.39% （80/83）. There was no significant difference between the two methods（P>0.05）.The positive detection rate of the control group was 86.59%（71/82）， which was lower than that of pathological results （96.34%， 79/82）. The difference between the two methods was statistically significant（P<0.05）. Conclusion Low-dose Multi-slice helical CT scan is effective in the diagnosis of acute appendicitis in children. It can observe the lesions in multiple directions and multiple planes，effectively improve the sensitivity and specificity of clinical diagnosis， and has high safety. It is worthy of popularization and application.

兒童急性闌尾炎屬于小兒外科常見急腹癥，年齡≤5歲的兒童，急性闌尾炎發病率占所有闌尾病例的15%～24%[1-2]。患兒早期無典型臨床癥狀，診斷漏診及誤診率較高[3-5]，而且往往手術和病理組織學檢查結果之間的差異很大[6]。因此，早發現、早診斷、早治療，具有重要臨床意義。臨床對于兒童急性闌尾炎的診斷，常規CT掃描診斷靈敏度及特異度較高;但常規射線計量對患兒的輻射較大[7-8]。多層螺旋CT低劑量掃描技術憑借強大的后處理功能及低輻射等優點，從不同角度觀察組織器官解剖結構，在兒童急性闌尾炎的診斷中被廣泛重視應用[9]。我院2016年12月至2020年12月共收治兒童急性闌尾炎患兒165例，采用多層螺旋CT低劑量掃描技術進行診斷，現報道如下。
