宮旁阻滯麻醉在臨床中應用價值

摘要：目的 探討宮旁神經阻滯麻醉在刮宮、放取環等婦科小手術中的應用價值。方法 對三種手術的多例患者進行宮旁神經阻滯麻醉，觀察術中的麻醉效果以及副反應。結果 刮宮術123例，有效與顯效者為120例（97.6%）；放取環158例，顯效與有效共156例（97.5%）。結論 宮旁神經阻滯麻醉具有良好的鎮痛效果，并具有安全、操作簡單、費用低廉、副反應少等優點，值得在缺少足夠的麻醉醫師的基層、中小醫院中推廣應用。

Analysis of Uterine Nerve Block Anesthesia Clinical Application Value

Abstract：Objective To investigate the uterine curettage nerve block anesthesia， put the value of the ring taking in other gynecological minor. Methods For Multi-three patients were surgical uterine nerve block anesthesia， intraoperative anesthetic effect and side effects. Results Curettage 123 cases， 120 were valid and effective in patients （97.6%）； take the ring to put 158 cases， a total of 156 cases were markedly effective （97.5%）. Conclusion Uterine nerve block anesthesia with good analgesia， and a safe， simple， low cost， less side effects， etc.， should be promoted in the absence of adequate primary anesthesiologist， small hospital applications.

近年來，意外妊娠逐年增加，輸卵管結扎術減少，使用宮內節育環避孕的婦女越來越多，使得人工流產術及放取環等婦科小手術數量大大增加。以往認為，此類手術時間短，勿需麻醉。隨著生活水平的提高，人們對醫療效果的要求越來越高，特別是要求減少手術中的疼痛。因此，如何解決術中患者的疼痛成為醫生關注的重要問題。對于醫務人員來說，適當的麻醉是手術順利進行的前提。婦科手術以盆腔或陰道內器官為對象，手術部位深，視野小，操作相對困難，若能有充分的鎮痛和完善的肌松，對醫患雙方均有利。我院收治的患者大多為周邊農村的農民及城鎮居民中的低中收入人群，因此，選擇費用低廉且安全有效的止痛方法尤為重要。為尋找良好的麻醉方法，我科也采用全身靜脈麻醉方式，但因費用較高，有一定的局限性。我科自2012年2月以來，對進行刮宮（包括人工流產、診斷性刮宮、藥物流產后刮宮）、放取環的患者采取宮旁神經阻滯麻醉，取得了良好的效果。……
