多房囊性腎瘤報告及文獻復習

摘要：目的 探討多房性囊性腎瘤的診斷與治療。方法 回顧性分析1例多房性囊性腎瘤的臨床資料，復習文獻予以討論。結果 行后腹腔鏡左腎部分切除術，術后病理檢查證實為多房性囊性腎瘤。隨訪至今未見復發。結論 多房囊性腎瘤是一種少見的腎臟良性腫瘤，確診有賴于病理學檢查，保留腎單位手術是本病的首選治療方法。

Abstract：Objective To study the diagnosis and treatment of multilocular cystic nephroma. Methods Retrospective analysis the clinical data of a case of the multilocular cystic nephroma，and review literatures to discuss.Results Underwent after laparoscopic of the left kidney partial nephrectomy， Postoperative pathological examination onfirmed for multilocular cystic nephroma.During the follow -up no evidence of recurrence is found till now.Conclusion Becuse multilocular cystic nephroma is a rare benign renal neoplasm.Diagnosis depends on pathological examination.Nephron sparing surgery is the primary and effective therapeutic method for this disease.

多房囊性腎瘤（multilocular cystic nephroma，MCN）是一種比較少見的腎臟良性腫瘤。我科于2013年11月收治1例患者，現結合文獻分析報告如下。

1臨床資料

患者，男，32歲，因\"體檢發現雙腎囊性占位7月余\"于2013年11月入院。體格檢查無陽性體征。門診資料：2013年3月彩超示雙側腎臟包膜光滑，皮髓質分界清晰，集合系統光點群無分離；右側腎臟中上部有約2.4 cm×2.0 cm大小囊性無回聲，邊界清，內透聲可，囊壁見強回聲斑；左側腎臟下極實質內有約3.1 cm×2.7 cm大小囊性無回聲，邊界清，內透聲差，見多條分隔光帶及數個強回聲斑；雙側輸尿管未見擴張。CDFI：雙側腎臟血流信號豐富，動、靜脈呈\"指狀\"分布。入院查CT平掃+增強示右側腎臟上部、左側腎臟下極各一囊狀低密度無強化影，低于腎臟實質密度，右側者大小約2.0 cm×2.4 cm，左側者形狀不規則，大小約2.4 cm×3.2 cm，小分隔強化，境界欠清；腎周結構正常，雙側腎盂、腎盞及輸尿管上段未見異常，雙腎動、靜脈顯影良好；肝臟、膽道系統、胰腺、脾臟未見異常；腹膜后未見腫大淋巴結。尿常規、血常規、出凝血時間、肝腎功能、電解質、血糖各值無異常。入院擬診為左側腎臟多房囊性占位性病變：MCN？……
