【摘要】目的:觀察分析超聲造影檢查在睪丸扭轉中的診斷價值。方法:選取2013年1月至2015年9月我院收治的經手術和臨床隨訪證實的睪丸扭轉患者共計66例,按照隨機數表法分為觀察組(n=33)和對照組(n=33),對照組應用常規超聲檢查,觀察組應用超聲造影檢查。觀察比較兩組患者診出睪丸扭轉的符合率、不同類型睪丸扭轉檢出率以及兩種檢查方法的超聲表現情況。結果:觀察組患者診出睪丸扭轉符合率(84.85%)明顯高于對照組患者診出睪丸扭轉符合率(42.42%),差異具有統計學意義(P<0.05)。同時,觀察組睪丸完全扭轉檢出率(51.52%)和睪丸不完全扭轉檢查率(33.33%)均高于對照組患者(12.12%、30.3%),數據差異具有統計學意義(P<0.05)。結論:超聲造影檢查診斷患者睪丸扭轉符合率高,同時能夠準確判定睪丸缺血受損的程度和范圍,為睪丸扭轉診斷以及評估提供重要影像學資料。

【Abstract】Objectives: To study the diagnostic value of contrast-enhanced ultrasound in testicular torsion. Methods: 66 cases with testicular torsion from January 2013 to September 2015 in our hospital were researched, and according to the random number table were divided into observation group (n=33) and control group (n=33). Control group were treated by Color Doppler ultrasonography, while observation group used CEUS. The diagnosed testicular torsion compliance rate, different types of testicular torsion detection rate and the performance of the two methods of ultrasound was compared. Results: The diagnosed testicular torsion compliance rate of observation group (84.85%) was significantly higher than the control group (42.42%), with statistically significant difference (P<0.05). At the same time, the testicular torsion complete detection rate (51.52%) and incomplete testicular torsion checking rate (33.33%) in observation group were higher than the control group (12.12%, 30.3%), with statistically significant difference (P<0.05). Conclusions: CEUS is consistent with the diagnosis of patients with high testicular torsion rate. At the same time, it can accurately determine the testicles missing blood damage extent and scope, which provides important image data for diagnosis and assessment of testicular torsion.

睪丸扭轉又稱為精索扭轉,其發病機制多因為睪丸和精索本身的活動度加大以及解剖異常而引發扭轉,初期扭轉以及輕度扭轉可以導致靜脈淤血腫脹以及回流出現障礙,嚴重的還能致使動脈供血出現障礙而引發睪丸缺血壞死。睪丸扭轉屬于泌尿科的嚴重急癥之一,同時也極其容易出現漏診和誤診,因此在早期準確、迅速的診斷出病癥能夠有效減少患者睪丸出現壞死的情況是搶救成功的關鍵[1,2]。臨床研究表明,超聲檢查對于治療前確定睪丸缺血受損的程度和范圍效果顯著,被認為是最為有效且安全的影像學檢查方法之一。但是其不足之處是在診治嬰幼兒患者時其睪丸血流信號相對較弱,再加上嬰幼兒患者在治療時的不配合,很大程度上會影響診治結果,導致漏診和誤診發生,影響患者的生命健康[3]。……
