【摘要】目的:觀察水通道蛋白9(AQP9)在子癇前期患者胎盤、胎膜組織中的定位、mRNA及表達變化,分析AQP9在子癇前期發病中發揮的作用。方法:選取本院2014年3月至2015年5月收治的100例孕婦作為研究對象,其中有50例為正常晚期妊娠孕婦,將這50例作為對照組;另外50例為子癇前期孕婦(孕周38~42周),輕度子癇前期孕婦與重度子癇前期孕婦各25例,將這50例子癇前期患者作為觀察組。利用免疫組織化學增強聚合物法對AQP9的定位情況進行測定;利用逆轉錄-聚合酶鏈反應對AQP9 mRNA進行檢測;利用蛋白印跡技術對蛋白的表達水平進行檢測;通過使用凝膠蛋白定量分析軟件對蛋白灰度值進行測定。結果:(1)AQP9在羊膜上皮細胞、絨毛膜滋養細胞和合體滋養細胞上定位;(2)兩組患者的胎膜組織和胎盤中都有AQP9 mRNA表達;(3)觀察組胎膜組織、胎膜中AQP9的灰度值都要高于對照組,差異顯著,具有統計學意義(P<0.05),而且觀察組中重度子癇前期患者胎膜組織、胎膜中AQP9的灰度值要高于輕度子癇前期患者,對比差異,具有統計學意義(P<0.05)。結論:子癇前期患者AQP9的表達水平得到較大程度的提升,AQP9是造成子癇前期發病的主要因素。

【Abstract】Objectives: To observe changes in the location, mRNA and expression of aquaporin-9 (AQP9) in placenta and fetal membrane of preeclampsia patients, to analyze its role in the pathogenesis of preeclampsia. Methods: 100 pregnant women treated in our hospital from March 2014 to May 2015 were selected including 50 normal third trimester pregnant women as control group and 50 preeclampsia patients (gestational age of 38 to 42 weeks) as observation group, i.e. 25 mild preeclampsia pregnant women and 25 severe ones. Immunohistochemical reinforced polymer was used to determine the location of AQP9, reverse transcription-polymerase chain reaction to test the 9 mRNA, protein imprinting technology to test the protein expression levels, and the gel protein quantitative analysis software for determination of protein grey value. Results: Firstly, the AQP9 was located in amniotic epithelial cells, human chorionic trophocyte and sertoli cells. Secondly, expression of AQP9 was found in the membranes and placenta of both groups. Thirdly, the AQP9 grey value on placenta and fetal membrane in the observation group was higher than the control group, with statistically significant difference (P<0.05), and the AQP9 grey value on placenta and fetal membrane in patients with severe preeclampsia was also higher than those with mild preeclampsia, with statistically significant difference (P<0.05). Conclusions: Expression level of AQP9 in patients with preeclampsia increases, indicating AQP9 is the main factor of preeclampsia.

孕婦在妊娠期間普遍會患上妊娠期高血壓疾病,而子癇前期是最常見的類型之一,同時也是造成孕產婦死亡的主要原因[1]。目前,子癇前期發病機制仍然處于不清晰的狀態,專家認為早期形成胎盤時,滋養細胞沒有深入入侵子宮內膜,重鑄子宮螺旋小動脈出現障礙,從而使胎盤出現“淺著床”現象,這些因素促使子癇前期的發作[2]。細胞分化及遷移過程中,水通道蛋白9(AQP9)會對其產生一定的影響,其表達升高在一定程度上加快了細胞凋亡的速度,細胞的遷移能力逐漸下降,細胞分化和細胞融合也會受到相應抑制作用,導致胎盤出現缺血缺氧現象,由此推斷子癇前期發病很有可能與AQP9異常表達有一定的關系?!?br>
