【摘要】目的:研究光動(dòng)力治療合并人類免疫缺陷病毒(HIV)感染的尖銳濕疣患者臨床效果。方法:回顧性分析于2012年1月到2015年1月在我院確診為尖銳濕疣合并HIV感染的24例患者。14例患者應(yīng)用5-氨基酮戊酸光動(dòng)力治療,10例患者應(yīng)用二氧化碳激光治療,分別比較兩組患者的完全緩解率、復(fù)發(fā)率、不良反應(yīng)率。結(jié)果:兩組患者的完全緩解率之間的差異無(wú)統(tǒng)計(jì)學(xué)意義(χ2=0.062,P=0.875);而光動(dòng)力治療組的復(fù)發(fā)率和不良反應(yīng)率分別為7.14%和21.43%,明顯低于二氧化碳激光治療組的復(fù)發(fā)率和不良反應(yīng)率(60.00%和80.00%),兩組在復(fù)發(fā)率和不良反應(yīng)率上差異具有統(tǒng)計(jì)學(xué)意義(χ2=5.538,P=0.019;χ2=7.837,P=0.005)。結(jié)論:光動(dòng)力治療法是可以有效地治療合并HIV感染的尖銳濕疣。

【Abstract】Objectives: To study the clinical effect of photodynamic therapy in treatment of HIV patients with condyloma acuminatum. Methods: 24 patients diagnosed with HIV and condyloma acuminatum at our hospital from January 2012 to January 2015 were retrospectively analyzed.14 patients were treated with photodynamic therapy as the photodynamic therapy group.10 patients treated with carbon dioxide laser therapy were included in the carbon dioxide laser therapy group. The rate of complete remission, recurernce, adverse reaction of the two groups was compared. Results: The rate of recurrence and adverse reaction of photodynamic therapy group were 7.14% and 21.43% respectively, obviously lower than these of the carbon dioxide laser therapy group (60.00% and 80.00%), both with significantly statistical difference (χ2=5.538, P=0.019;χ2=7.837, P=0.005). Conclusion: Photodynamic therapy can effectively treat HIV infected patients with condyloma acuminatum.

1前言
尖銳濕疣(CA),又名肛門生殖器疣、性病疣,主要由人類乳頭狀瘤病毒(HPV)感染所致的傳染性極強(qiáng)的性傳播疾病,HPV的潛伏期為1~8個(gè)月,平均約為3個(gè)月,傳播方式以性傳播為主,好發(fā)于生殖器、肛周等性接觸部位,在我國(guó)性傳播疾病中高居第三位[1]。隨著近年來(lái)男男性行為(MSM)的增多,這不僅引起CA的發(fā)病率進(jìn)一步上升,還導(dǎo)致人類免疫缺陷病毒(HIV)感染率的上升[2]。由于免疫力降低,CA合并HIV感染的患者因疾病頻繁復(fù)發(fā)導(dǎo)致療效不佳,另一項(xiàng)可能的原因是傳統(tǒng)治療方法如激光、電刀、微波、冷凍等只能對(duì)肉眼所見(jiàn)的疣進(jìn)行切割,而對(duì)處于HPV潛伏期或亞臨床感染卻無(wú)作用,而光動(dòng)力治療(PDT)是近年發(fā)展的一種新型治療方法,通過(guò)光敏劑在病毒感染病灶的聚集,高度選擇性破壞病變組織,可有效地降低CA的復(fù)發(fā)率[3]。……
