加巴噴丁聯合氟芬那酸丁酯軟膏治療帶狀皰疹后遺神經痛48例臨床觀察

【摘要】 目的：評價加巴噴丁聯合氟芬那酸丁酯軟膏治療帶狀皰疹后遺神經痛的臨床效果。方法：將入選的94例患者隨機分為兩組，治療組

（48例）每日分3次口服加巴噴丁300～900 mg，最大1200 mg，同時予氟芬那酸丁酯軟膏均勻外搽皮損，3次/d。對照組（46例）口服卡馬西平0.1 g，消炎痛25 mg，均3次/d。兩組療程均為4周。結果：治療4周后，治療組總有效率為87.50%，對照組為69.57%，差異有統計學意義（字2=4.51，P<0.05）。兩組治療前VAS評分比較差異無統計學意義（t=0.578，P>0.05），治療4周后，治療組低于對照組，疼痛緩解程度較大，差異有統計學意義（t=11.190，P<0.05）。兩組患者治療過程和隨訪期間，無不良反應發生。結論：帶狀皰疹后遺神經痛患者，加巴噴丁聯合氟芬那酸丁酯軟膏應用，能迅速緩解疼痛癥狀，顯著提升患者生活質量，不良反應較少，臨床值得運用。

【Abstract】 Objective：To investigate the clinical effects of Butyl Flufenamate Ointment combined with Gabapentin in patients with post-herpetic neuralgia（PHN）.Method：94 patients with PHN were randomized divided into treatment group（n=48） and control group（n=46）.The treatment group received oral Gabapentin （300-900 mg） with a maximum dose of 1200 mg，3 times a day，besides Butyl Flufenamate Ointment was applied to the skin lesionevenly，3 times a day.The control group received oral Carbamazepine（100 mg） and Indomethacin（25 mg），3 times a day.Both groups were treated for 4 weeks.Result：After 4 weeks of treatment，the total effective rate of the treatment group was 87.50%，while the control group was 69.57%，the difference was statistically significant（字2=4.51，P<0.05）.Before the treatment，the difference in VAS in both groups showed no statistical significance（t=0.578， P>0.05）.After 4 weeks of treatment，the VAS of the treatment group was lower than that of the control group，and the difference was statistically significant（t=11.190，P<0.05）.There was no adverse reaction between the two groups during the course of treatment and follow-up.Conclusion：Butyl Flufenamate Ointment combined with Gabapentin can relieve the symptoms of pain in patients with post-herpetic neuralgia and improve the quality of life with less adverse effects， and it is worth using clinically.

帶狀皰疹后遺神經痛（postherpetic neuralgia，PHN）患者常常因為異常性疼痛和燒灼樣疼痛，生活質量顯著下降，加巴噴丁作為抗癲癇藥物之一，同時也具有顯著緩解疼痛的作用，常被推薦為治療PHN的首選藥物[1]。本研究旨在分析探討筆者所在醫院2009年7月-2012年5月對于48例帶狀皰疹后遺神經痛患者，采用加巴噴丁聯合氟芬那酸丁酯軟膏治療效果，現總結如下。

1.1 一般資料

所有患者治療前均知情同意，并簽署知情同意書：（1）經過筆者所在醫院或外院確診的帶狀皰疹患者；（2）接受過規范治療，皰疹已完全消退，遺留神經痛者，且疼痛時間持續>1個月[2]。排……
