280例多發傷患者的急診救治分析

【摘要】 目的：分析多發傷患者的急診救治措施與效果。方法：選取筆者所在醫院于2013年5月-2016年5月收治的280例多發傷患者，回顧性分析其臨床資料，總結急診救治措施與臨床結局。結果：280例多發傷患者的搶救成功率為93.57%（262/280），18例患者死亡，死亡率為6.43%。主要致死原因包括高位脊髓損傷、多器官功能衰竭、嚴重心肺損傷、嚴重肝脾破裂傷、特重型顱腦損傷等。死亡時間分布：入院前死亡5例，搶救室死亡4例，術后死亡9例。結論：急診科應及時、準確地評估病情，確定搶救順序，把握最佳急救時機，采取一體化救治措施，確保患者的生命安全。

【Abstract】 Objective：To analyze the emergency treatment measures and effects of multiple trauma patients.Method：280 cases with multiple injuries patients clinical data were selected，they were treated in our hospital from May 2013 to May 2016，and the emergency treatment measures and clinical outcomes were summarized.Result：The success rate of rescue in 280 cases with multiple injuries was 93.57%（262/280），18 patients died，and the death rate was 6.43%.The main causes of death including spinal cord injury，multiple organ failure，severe heart lung injury，severe liver and spleen rupture，severe craniocerebral injury.Time distribution of death：5 cases died before admission，4 cases died in resuscitation room，9 cases died after operation.Conclusion：Emergency department shall be timely and accurate assessment of the disease，determine the order of the rescue，grasp the best time to first aid，take an integrated treatment measures to ensure the safety of the patients life.

機動車數量的增多雖然為人們的生活帶來了便利，但是同時也引起了交通擁堵等問題，我國的交通事故發生率居高不下，由此而造成的多發傷也不斷增加。另外，還有一些意外災害，如工傷、高處墜落等也會導致多發傷，患者不僅病情嚴重，而且進展迅速，一旦未得到及時救治，很容易致殘或者致死[1]。筆者所在醫院分析了多發傷患者的急診救治措施及效果，報道如下。

1 資料與方法

1.1 一般資料

選取筆者所在醫院于2013年5月-2016年5月收治的多發傷患者280例，男163例，女117例；年齡13～80歲，平均（41.6±4.8）歲；致傷原因：火器傷1例，擠壓傷8例，嚴重燒傷9例，爆炸傷6例，斗毆刀傷28例，機械碾軋傷31例，高空墜落傷38例，交通事故傷159例；受傷部位數量：2個部位受傷26例，3個部位受傷132例，4個部位受傷99例，5個及以上部位受傷23例；病情嚴重程度（用T1，即創傷指數評估）：輕傷28例，重度傷56例，危重傷149例，極危重傷47例。……
