[摘要] 目的 探討miRNA-1表達水平和心肺復蘇后心功能障礙嚴重程度的相關性。 方法 選擇2014年2月~2016年2月期間在本院急診科心肺復蘇成功的心臟驟停患者62例作為病例組,按年齡、性別匹配取62例健康自愿者作為對照組,采用Real-time PCR檢測miRNA-1表達水平。觀察病例組患者超聲心動圖心功能各項指標,分析miRNA-1表達水平與心功能障礙嚴重程度的相關性。 結果 病例組miRNA-1表達水平明顯高于對照組(P<0.05),心功能Ⅳ級組miRNA-1表達水平明顯高于心功能Ⅲ級組(P<0.05),miRNA-1表達水平與心功能指標E峰、E/A值、LVEF、LVEDD呈負相關(P<0.05),miRNA-1表達水平與心功能障礙嚴重程度呈正相關(r=0.864, P<0.01)。 結論 miRNA-1參與了心肺復蘇后心肌細胞凋亡的發生和發展,與心功能障礙嚴重程度密切相關,其具體途徑可能是通過調節靶基因從而促進或抑制靶基因蛋白表達而實現。

Correlation study of miRNA-1 and severity of cardiac dysfunction after cardiopulmonary resuscitation

[Abstract] Objective To investigate the correlation between the expression level of miRNA-1 and the severity of cardiac dysfunction after cardiopulmonary resuscitation. Methods 62 patients with cardiac arrest who were given cardiopulmonary resuscitation in the Emergency Department in our hospital from February 2014 to February 2016 were assigned to the subject group. 62 healthy volunteers were assigned to the control group by age and gender. The expression level of miRNA-1 was tested by real-time PCR. The indices of cardiac function were observed in the subject group by echocardiography, and the correlation between the expression level of miRNA-1 and the severity of cardiac dysfunction was analyzed. Results The expression level of miRNA-1 in the subject group was significantly higher than that in the control group (P<0.05). The expression level of miRNA-1 in the cardiac function level Ⅳ group was significantly higher than that in the cardiac function level Ⅲ group(P<0.05). The expression level of miRNA-1 was negatively correlated with E-peak, E/A value, LVEF and LVEDD(P<0.05). The expression level of miRNA-1 was positively correlated with the severity of cardiac dysfunction(r=0.864, P<0.01). Conclusion miRNA-1 participates in the occurrence and progression of myocardial apoptosis after cardiopulmonary resuscitation, and it is closely related to the severity of cardiac dysfunction. The specific pathway may be obtained by modulating the target gene in order to promote or inhibit the expression of the target gene protein.
