人工乳頭配合手法操作協(xié)助乳頭凹陷產(chǎn)婦哺乳的臨床應(yīng)用

摘 要：目的 探討人工乳頭配合手法在乳頭凹陷產(chǎn)婦哺乳中的臨床應(yīng)用效果。方法 選取2017年5月～ 12月在本院收治接受治療的120例乳頭凹陷的產(chǎn)婦，按照隨機(jī)數(shù)字表法將其分為研究組和對照組，每組60例。研究組采用人工乳頭配合手法進(jìn)行治療，對照組僅采用人工乳頭進(jìn)行治療。觀察兩組患者發(fā)生乳汁淤積和乳腺炎的情況，比較兩組母乳喂養(yǎng)效果和患者滿意度。結(jié)果 研究組發(fā)生乳汁淤積（3.34% vs18.34%）和乳腺炎（1.67% vs 15.00%）的人數(shù)較對照組少，差異具有統(tǒng)計(jì)學(xué)意義（P<0.05）。研究組60例產(chǎn)婦中有56例產(chǎn)婦成功的進(jìn)行了母乳喂養(yǎng)，4例患者由于新生兒哭鬧以及家屬配合力不高終止母乳喂養(yǎng)，其母乳喂養(yǎng)成功率（93.34%）高于對照組（65.00%），研究組產(chǎn)婦滿意度（96.67%）高于對照組（78.34%），兩組比較差異有統(tǒng)計(jì)學(xué)意義（P<0.05）。結(jié)論 對乳頭凹陷產(chǎn)婦實(shí)施人工乳頭配合手法按摩在母乳喂養(yǎng)中臨床療效顯著，有效提高了母乳喂養(yǎng)的成功率，降低產(chǎn)婦乳腺炎的發(fā)生，提高了患者的滿意度。

Clinical Application of Artificial Nipple Combined with Manual Manipulation to Assist Parturient Breast-feeding with Nipple Depression

Abstract：Objective To explore the clinical application of artificial nipple combined with manual manipulation in parturient with nipple depression. Methods 120 cases of parturient with nipple depression who were treated in our hospital from May to December 2017 were selected and divided into study group and control group with 60 cases in each group.The study group was treated with artificial nipple combined with manual manipulation， while the control group was treated only with artificial nipple. To observe the occurrence of milk siltation and mastitis in the two groups， and compare the effect of breast feeding and patients' satisfaction.Results The number of patients with milk deposition （3.34% vs 18.34%） and mastitis （1.67% vs 15.00%） was lower in the study group than in the control group，the difference was statistically significant（P<0.05）. 56 of the 60 women in the study group were successfully breastfed， and 4 patients stopped breastfeeding due to neonatal crying and low family coordination. The success rate of breastfeeding （93.34%） was higher than that of the control group （65.00%）， the maternal satisfaction rate of the study group（96.67%） was higher than that of the control group （78.34%）， the difference between the two groups was statistically significant （P<0.05）. Conclusion The artificial nipple combined with manual massage in the nipple depression has a significant clinical effect in breastfeeding， which effectively improves the success rate of breastfeeding， reduces the incidence of maternal mastitis， and improves patient satisfaction.

母乳喂養(yǎng)（breast feeding）是人類最原始也是最科學(xué)有效的喂養(yǎng)方法，它不僅對嬰兒有利而且對母親也有利[1]，能夠保護(hù)嬰兒健康使嬰兒免受感染、腹瀉、耳炎、過敏性疾病的侵襲，預(yù)防肥胖、高血壓、糖尿病等，還能促進(jìn)母親子宮恢復(fù)，減少產(chǎn)后出血[2，3]。然而一些產(chǎn)婦因?yàn)槿轭^凹陷不能進(jìn)行母乳喂養(yǎng)，這不僅造成資源浪費(fèi)而且還導(dǎo)致乳房脹痛和乳腺炎，并且影響女性美觀[4]。乳頭凹陷是較為常見的女性乳房畸形，其發(fā)病率約為10%[5，6]。現(xiàn)目前臨床上有各種治療的手法和器具，比如：乳頭保護(hù)貼、自制乳頭隆起器和抽吸奶嘴等。但是實(shí)踐證明這些方法的治療效果不甚理想。所以為了糾正產(chǎn)婦乳頭凹陷問題，提高母乳喂養(yǎng)率，本研究對人工乳頭配合手法在乳頭凹陷產(chǎn)婦中的臨床應(yīng)用效果進(jìn)行總結(jié)，報(bào)告如下。

1資料與方法
