[摘要] 目的 分析重癥肺炎患者血漿可溶性髓系細(xì)胞表達(dá)觸發(fā)受體-1(sTREM-1)水平變化及臨床意義。 方法 選取2018年6月~2019年3月在廣東醫(yī)科大學(xué)附屬第二醫(yī)院重癥監(jiān)護(hù)室治療的重癥肺炎24例(設(shè)為重癥肺炎組),其中有15例病情好轉(zhuǎn)(好轉(zhuǎn)組),選取健康體檢者19例作為對(duì)照組,采用ELISA方法檢測(cè)重癥肺炎組入住ICU第1天、好轉(zhuǎn)組病情穩(wěn)定轉(zhuǎn)至普通病房當(dāng)天及對(duì)照組體檢當(dāng)天的sTREM-1水平,并記錄重癥肺炎患者入住ICU第1天及其轉(zhuǎn)出病房當(dāng)天的急性生理學(xué)與慢性健康狀況(APACHE Ⅱ)評(píng)分。比較各組sTREM-1水平差異,統(tǒng)計(jì)分析sTREM-1與APACHE Ⅱ評(píng)分的相關(guān)性。 結(jié)果? 血漿sSTREM-1水平在重癥肺炎組、好轉(zhuǎn)組和對(duì)照組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05)。重癥肺炎組APACHE Ⅱ評(píng)分較好轉(zhuǎn)組明顯升高,差異有統(tǒng)計(jì)學(xué)意義(P<0.05)。重癥肺炎患者第1天血漿sTREM-1水平與APACHⅡ評(píng)分無(wú)明顯相關(guān)性(r=-2.53,P=0.223)。 結(jié)論 sTREM-1用于重癥肺炎的病情評(píng)估、預(yù)后判斷尚需持謹(jǐn)慎態(tài)度。

Value of soluble myeloid cells triggering receptor-1 expression in the assessment of severe pneumonia

[Abstract] Objective To analyze the changes of plasma soluble myeloid cells triggering receptor-1 (sTREM-1) levels and their clinical significance in patients with severe pneumonia. Methods 24 patients with severe pneumonia from June 2018 to March 2019 in the ICU of the Second Affiliated Hospital of Guangdong Medical University were selected and set as severe pneumonia group, of whom 15 cases were improved(improved group). 19 healthy physical examination cases were selected as the control group. STREM-1 levels of the severe pneumonia on the first day of ICU, on the day of stable transfer to the general ward of the improved group and on the day of the physical examination of the control group were detected by ELISA method. The acute physiology and chronic health status (APACHE Ⅱ) scores of patients with severe pneumonia on the first day of admission to the ICU and the day they were transferred out of the ward were recorded. The differences of sTREM-1 level in each group were compared, and the correlation between sTREM-1 and APACHE Ⅱ score was statistically analyzed. Results There was no statistically significant difference of plasma sTREM-1 level among the severe pneumonia group, the improved group and the control group(P>0.05). The APACHE Ⅱ score in the severe pneumonia group was obviously higher than that in the improved group, with statistically significant difference(P<0.05). There was no significant correlation between plasma sTREM-1 level and APACHⅡ score on the first day in patients with severe pneumonia(r=-2.53, P=0.223). Conclusion sTREM-1 should be used with caution in assessing the condition and prognosis of severe pneumonia.

重癥肺炎是一種進(jìn)展性肺部炎癥,短時(shí)間內(nèi)進(jìn)展迅速,且可引起腎臟、神經(jīng)系統(tǒng)等并發(fā)癥,病死率高,嚴(yán)重危及患者的生命安全[1]。早診斷和早治療十分重要,可降低死亡率,所以尋找特異性和敏感性高的生化指標(biāo),對(duì)病情的評(píng)估及預(yù)后判斷有著重要意義。目前臨床上常用白細(xì)胞和C-反應(yīng)蛋白等指標(biāo)對(duì)疾病嚴(yán)重程度及其預(yù)后進(jìn)行評(píng)估,但敏感性和特異性較差。髓系細(xì)胞觸發(fā)受體-1(TREM-1)是新近發(fā)現(xiàn)的一種炎癥激發(fā)受體,當(dāng)機(jī)體受炎癥感染時(shí)在中性粒細(xì)胞及單核細(xì)胞表面呈高表達(dá)狀態(tài),能促進(jìn)炎性因子分泌、誘導(dǎo)促炎介質(zhì)以及抑制抗炎因子產(chǎn)生,其介導(dǎo)的信號(hào)轉(zhuǎn)導(dǎo)在炎癥反應(yīng)的發(fā)生和級(jí)聯(lián)放大中起重要作用[2],能夠在感染期間進(jìn)入血液,與機(jī)體感染程度及預(yù)后有一定相關(guān)性[3]。本文觀(guān)察sTREM-1及APACHE Ⅱ評(píng)分在重癥肺炎中的變化及其之間的關(guān)系,探討其臨床價(jià)值,以指導(dǎo)臨床診療,現(xiàn)報(bào)道如下。
1 資料與方法
1.1 一般資料
選取2018年6月~2019年3月在廣東醫(yī)科大學(xué)附屬第二醫(yī)院重癥監(jiān)護(hù)室治療的重癥肺炎24例作為重癥肺炎組,其中15例作為病情好轉(zhuǎn)(好轉(zhuǎn)組)。其中男11例,女13例,平均年齡(69.3±4.7)歲。另選19例健康體檢者作為對(duì)照組,男11例,女8例,平均年齡(62.2±8.2)歲。各組受試者在性別、年齡方面的比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),具有可比性。所有患者符合重癥CAP的診斷標(biāo)準(zhǔn)[4]:符合下列1項(xiàng)主要標(biāo)準(zhǔn)或≥3項(xiàng)次要標(biāo)準(zhǔn)者可診斷為重癥肺炎,需密切觀(guān)察,積極救治,有條件時(shí)收住ICU治療(ⅡA)。主要標(biāo)準(zhǔn):(1)需要?dú)夤懿骞苄袡C(jī)械通氣治療;(2)膿毒癥休克經(jīng)積極液體復(fù)蘇后仍需要血管活性藥物治療。次要標(biāo)準(zhǔn):(1)呼吸頻率≥30次/min;(2)氧合指數(shù)≤250 mmHg(1 mmHg=0.133 kPa);(3)多肺葉浸潤(rùn);(4)意識(shí)障礙和(或)定向障礙;(5)血尿素氮≥7.14 mmol/L;(6)收縮壓<90 mmHg需要積極的液體復(fù)蘇。對(duì)照組均排除呼吸系統(tǒng)疾病、心腦血管疾病及急慢性感染者。本研究通過(guò)醫(yī)院倫理委員會(huì)批準(zhǔn),所有研究對(duì)象知情同意。好轉(zhuǎn)組入選標(biāo)準(zhǔn):重癥肺炎患者病情好轉(zhuǎn),氧合改善,可以撤有創(chuàng)呼吸機(jī)輔助通氣,休克糾正且停用血管活性藥物,酸堿平衡糾正等,好轉(zhuǎn)組患者已不再符合重癥肺炎診斷標(biāo)準(zhǔn)是最基本的入選標(biāo)準(zhǔn)。
1.2 方法
1.2.1 血漿sTREM-1水平測(cè)定? 重癥肺炎組于入院第1天,好轉(zhuǎn)組于病情穩(wěn)定轉(zhuǎn)出普通病區(qū)當(dāng)天檢測(cè)血漿中sTREM-1的表達(dá)。對(duì)照組于體檢當(dāng)天抽靜脈血5 mL。4℃,3500 r/min離心15 min,取上清液,將其置于-20℃冰箱凍存被檢。使用ELISA法測(cè)定血漿人可溶性髓系細(xì)胞觸發(fā)受體-1(sTREM-1),ELISA試劑盒購(gòu)于上海勁馬實(shí)驗(yàn)設(shè)備有限公司。
1.2.2 急性生理學(xué)與慢性健康(APACHE Ⅱ)評(píng)分[5]? 急性生理學(xué)與慢性健康(APACHE Ⅱ)評(píng)分,即由急性生理評(píng)分、年齡評(píng)分及慢性健康評(píng)分組成。其中急性生理評(píng)分包括體溫、平均動(dòng)脈壓、心率、呼吸頻率、氧分壓、血?dú)鈖H值或碳酸氫根濃度、血鉀、血鈉、血肌酐、白細(xì)胞比容、白細(xì)胞計(jì)數(shù)、格拉斯哥昏迷評(píng)分(GCS)在內(nèi)的12項(xiàng)生理指標(biāo);慢性健康評(píng)分要求患者符合慢性器官功能不全或免疫功能抑制,內(nèi)容包括肝臟功能、心血管、呼吸系統(tǒng)、腎臟功能及免疫功能抑制情況。最終APACHE Ⅱ評(píng)分等于急性生理評(píng)分、年齡評(píng)分與慢性健康評(píng)分之和。重癥肺炎患者在入院第1天和好轉(zhuǎn)組于病情穩(wěn)定轉(zhuǎn)普通病區(qū)當(dāng)天作急性生理學(xué)與慢性健康(APACHEⅡ)評(píng)分[5]。
1.3統(tǒng)計(jì)學(xué)分析
采用SPSS17.0統(tǒng)計(jì)學(xué)軟件處理數(shù)據(jù)。符合正態(tài)分布的計(jì)量資料以均數(shù)±標(biāo)準(zhǔn)差(x±s)表示,不符合正態(tài)分布的計(jì)量資料采用中位數(shù)(四分位間距)M(P25,P75)表示;對(duì)于符合正態(tài)分布的計(jì)量資料組間比較采用t檢驗(yàn);不符合正態(tài)分布的采用非參數(shù)秩和檢驗(yàn),兩個(gè)獨(dú)立樣本用Mann-Whitney檢驗(yàn),兩個(gè)相關(guān)樣本采用Wilcoxon檢驗(yàn);兩變量符合雙變量聯(lián)合正態(tài)分布,采用Pearson相關(guān)分析,不滿(mǎn)足Pearson相關(guān)分析條件的采用Spearman相關(guān)分析,P<0.05為差異有統(tǒng)計(jì)學(xué)意義。
2 結(jié)果
2.1 重癥肺炎組與對(duì)照組的血漿中sTREM-1比較
重癥肺炎組血漿sTREM-1水平與對(duì)照組比較,差異無(wú)統(tǒng)計(jì)學(xué)意義(Z=-1.211,P=0.226),見(jiàn)表1。
2.2 重癥肺炎組與好轉(zhuǎn)組血漿中sTREM-1、APACHE Ⅱ評(píng)分比較
重癥肺炎組APACHE Ⅱ評(píng)分較好轉(zhuǎn)組明顯升高,差異有統(tǒng)計(jì)學(xué)意義(P<0.05);重癥肺炎組血漿中sTREM-1與好轉(zhuǎn)組比較差異無(wú)統(tǒng)計(jì)學(xué)意義(P>0.05),見(jiàn)表2。

重癥肺炎組患者sTREM-1與APACHE Ⅱ評(píng)分無(wú)明顯相關(guān)性(r=-2.53,P=0.223)。
3 討論
重癥肺炎屬于一種全身性的炎性反應(yīng),肺部感染嚴(yán)重,由于受到病原體、炎性因子、毒性代謝產(chǎn)物的侵害,重癥肺炎具有發(fā)病迅速、治療難度大等特點(diǎn),可出現(xiàn)意識(shí)障礙、休克,導(dǎo)致多臟器功能衰竭,其產(chǎn)生與病原體感染后體內(nèi)瀑布樣炎性反應(yīng)及抗炎反應(yīng)失代償相關(guān),使局部炎癥向全身性炎癥演變,而非單純病原菌和毒素?fù)p傷的結(jié)果。細(xì)胞因子介導(dǎo)的炎癥反應(yīng)在重癥肺炎的發(fā)生、發(fā)展及轉(zhuǎn)歸中發(fā)揮著重要的作用。雖然在危重患者肺部感染的診斷和治療方面已經(jīng)取得了很大的進(jìn)展,但為了降低發(fā)病率和死亡率,仍需要尋找更準(zhǔn)確的生物標(biāo)志物來(lái)診斷肺部感染及判斷預(yù)后。
髓系細(xì)胞觸發(fā)受體-1(TREM-1)屬于免疫球蛋白超家族,于2000年被首次發(fā)現(xiàn)[6]。可溶性髓樣細(xì)胞觸發(fā)受體-1(sTREM-1)是TREM-1的可溶性形式,其作為調(diào)節(jié)因子在炎癥反應(yīng)中起著重要作用,TREM-1在細(xì)胞外細(xì)菌、真菌或炎癥介質(zhì)刺激時(shí)表達(dá)增加[7-8],因此可作為診斷感染性疾病的標(biāo)志物[9-11]。肺炎患者肺泡灌洗液及膿毒癥患者血漿內(nèi)sTREM-1水平均出現(xiàn)增高。研究表明,sTREM-1可作為重癥肺炎嚴(yán)重程度評(píng)估指標(biāo),在重癥肺炎患者血清中高表達(dá),可作為重癥肺炎的輔助診斷[12-13]。Gibot S等[14]研究認(rèn)為,sTREM-1血清水平可有效預(yù)警SIRS和膿毒癥,其價(jià)值高于PCT及CRP。陳明科等[15]研究認(rèn)為,腹腔引流液sTREM-1水平對(duì)腹部創(chuàng)傷膿毒癥的診斷具有參考價(jià)值,動(dòng)態(tài)監(jiān)測(cè)腹腔引流液sTREM-1水平變化,有利于評(píng)估腹部創(chuàng)傷膿毒癥的治療效果和判斷預(yù)后。Grover V等[16]研究發(fā)現(xiàn),單核細(xì)胞和中性粒細(xì)胞上表達(dá)TREM-1(mTREM-1)水平, BALF中sTREM-1水平較無(wú)膿毒癥證據(jù)的通氣對(duì)照組與無(wú)臨床感染的非通氣對(duì)照組明顯升高,認(rèn)為包含sTREM-1在內(nèi)的7-標(biāo)記生物核可以準(zhǔn)確區(qū)分VAP和非肺部感染。
但本研究卻發(fā)現(xiàn),重癥肺炎組患者血漿sTREM-1水平較對(duì)照組患者無(wú)明顯升高,差異無(wú)統(tǒng)計(jì)學(xué)意義;sTREM-1水平在病情好轉(zhuǎn)組表達(dá)較重癥肺炎組并沒(méi)有明顯下降。重癥肺炎組患者血漿sTREM-1水平與APACHE Ⅱ評(píng)分無(wú)明顯的相關(guān)性。這結(jié)果不排除其他干擾因素所致,如入選患者在入院前可能已接受抗感染或激素治療;不同的致病菌可能會(huì)影響sTREM-1表達(dá)水平,故尚不能說(shuō)明sTREM-1在重癥肺炎發(fā)生、發(fā)展不起作用。當(dāng)然也有研究發(fā)現(xiàn)sTREM-1并不能反映疾病嚴(yán)重程度。Bopp C等[17]研究發(fā)現(xiàn),血漿中sTREM-1表達(dá)水平在SIRS組、重度膿毒癥組及膿毒癥休克組較對(duì)照組并沒(méi)有升高,生存組和死亡組中血漿sTREM-1水平比較差異無(wú)統(tǒng)計(jì)學(xué)意義。一項(xiàng)納入11項(xiàng)研究共1795例患者的Meta分析[18]顯示,sTREM-1在區(qū)分SIRS 和膿毒癥方面有一定幫助,然而血漿sTREM-1作為單一指標(biāo)診斷膿毒癥并不可靠。在新生兒遲發(fā)型敗血癥中sTREM-1的診斷價(jià)值不如IL-6[19]。最近Esposito S等[20]研究表明,社區(qū)獲得性肺炎患兒血中sTREM-1水平難以鑒別是細(xì)菌還是病毒感染,對(duì)嚴(yán)重病例的鑒別能力較差。另外一項(xiàng)系統(tǒng)分析[21],就血漿sTREM-1水平在診斷兒童全身性炎癥反應(yīng)綜合征(SIRS)膿毒癥中的準(zhǔn)確性進(jìn)行系統(tǒng)回顧評(píng)價(jià),該系統(tǒng)分析了包括961例患兒在內(nèi)的9項(xiàng)研究,結(jié)果發(fā)現(xiàn)目前的資料不足以支持sTREM-1在兒童膿毒癥中的診斷價(jià)值。sTREM-1作為重癥肺炎或膿毒癥的準(zhǔn)確預(yù)警和判斷預(yù)后的指標(biāo)需持謹(jǐn)慎態(tài)度。
綜上所述,血漿sTREM-1水平在重癥肺炎組、好轉(zhuǎn)組、對(duì)照組中差異無(wú)統(tǒng)計(jì)學(xué)意義,各組的sTREM-1水平與APACHE Ⅱ評(píng)分均無(wú)相關(guān)性,sTREM-1用于重癥肺炎的病情評(píng)估、預(yù)后判斷需持謹(jǐn)慎態(tài)度;或者說(shuō)sTREM-1作為單一指標(biāo)診斷評(píng)價(jià)重癥肺炎并不可靠,應(yīng)從臨床各個(gè)方面,結(jié)合相關(guān)輔助檢查及生物標(biāo)志物綜合評(píng)估重癥肺炎病情及預(yù)后。
