益氣活血法治療糖尿病腎病的Meta分析

[摘要] 目的 探討益氣活血法治療糖尿病腎病的臨床療效。 方法 檢索通過檢索萬方期刊數據庫、中國期刊全文數據庫（CNKI）、中文期刊數據庫（維普），檢索時間限定為建庫至2018年10月，檢索所有具有益氣活血法治療糖尿病腎病的隨機對照試驗，同時手工輔助檢索相關核心期刊，追索納入研究的參考文獻。2名研究者對文獻、資料進行單獨篩選與提取，使用軟件Rev Man 5.3作Meta分析。 結果 納入30篇中文RCT，5篇論文評定為B級，25篇評定為C級。其中6個試驗采用了隨機對照試驗。Meta分析結果顯示：與基礎治療比較，益氣活血法可顯著降低尿微量蛋白排泄率[WMD=41.25，95%CI（21.25，37.56），P<0.05];可顯著提高患者臨床總有效率[Z=10.25，OR=5.21，95%CI（3.45/7.25）]，差異有統計學意義（P<0.05）。 結論 益氣活血法治療糖尿病腎病療效顯著，值得推廣。

[Abstract] Objective To investigate the clinical efficacy of Yiqihuoxue therapy in the treatment of diabetic nephropathy. Methods Search through the retrieval of Wanfang Journal Database， China Journal Full-text Database （CNKI）， Chinese Journal Database （VIP）， the search time was limited from establishing database until October 2018， and all randomized controlled trials with Yiqihuoxue therapy for diabetic nephropathy were searched. At the same time， manual assisted retrieval of relevant core journals， retrieving references included in the study. In the two studies， the literature and data were screened and extracted separately， and the software Rev Man 5.3 was used for meta-analysis. Results 30 Chinese RCTs were included. 5 papers were rated as Grade B and 25 were rated as Grade C. 6 of the trials used randomized controlled trials. Meta-analysis results showed that compared with the basic treatment， Yiqihuoxue method can significantly reduce the micro-protein excretion rate of urine [WMD=41.25， 95% CI （21.25， 37.56）， P<0.05]; can significantly improve the clinical total effective rate of patients [Z=10.25，OR=5.21， 95%CI （3.45/7.25）]， the difference was statistically significant（P<0.05）. Conclusion The effect of Yiqihuoxue therapy on diabetic nephropathy is significant and worthy of promotion.

糖尿病腎病是糖尿病患者后期常見并發癥之一，發病率受飲食、生活發生因素的影響，呈上升趨勢，是終末期腎臟病的主要原因之一，僅次于腎小球炎[1-2]。糖尿病腎病患者，多具有復雜代謝絮亂顯現，在疾病發展至終末期腎臟病時，難以進行有效治療，因此早期防治對延緩病情具有重要意義[3-4]。發病機制上不清楚，多認為是由多因素參與，臨床治療以藥物為主，中醫在治療糖尿病腎病上，可降低不良反應，提升治療效果[5]。該研究運用循證醫學方法原理，為系統性評價益氣活血法治療糖尿病腎病的有效性，全面搜索有關益氣活血法治療糖尿病腎病的隨機對照臨床研究，旨在為臨床治療提供循證依據。報道如下。
