卡托普利聯(lián)合肝素鈉對(duì)腎病綜合征患者腎功能及血液流變學(xué)指標(biāo)的影響

【摘要】 目的：探討卡托普利聯(lián)合肝素鈉對(duì)腎病綜合征患者腎功能及血液流變學(xué)指標(biāo)的影響。方法：選取2018年4月-2020年3月本院收治的腎病綜合征患者92例作為研究對(duì)象，用信封法將患者分為對(duì)照組和研究組，各46例。對(duì)照組采用常規(guī)治療，研究組采用卡托普利聯(lián)合肝素鈉治療。對(duì)比兩組患者的血漿白蛋白、24 h尿蛋白量及血尿素氮、血脂指標(biāo)、腎功能指標(biāo)、血液流變學(xué)指標(biāo)和臨床治療效果。結(jié)果：治療后，研究組的血漿白蛋白水平高于對(duì)照組、24 h尿蛋白定量低于對(duì)照組，差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）;研究組總有效率為87.0%，高于對(duì)照組的60.9%，差異有統(tǒng)計(jì)學(xué)意義（P<0.05）;治療后，研究組的HDL-C水平高于對(duì)照組，TC、TG、LDL-C水平均低于對(duì)照組（P<0.05）;治療后，研究組Scr及BUN水平均低于對(duì)照組，差異均有統(tǒng)計(jì)學(xué)意義（P<0.05）;治療后，研究組纖維蛋白原、全血黏度高切及低切、血漿黏度、D-二聚體水平均低于對(duì)照組（P<0.05）。結(jié)論：卡托普利聯(lián)合肝素鈉治療腎病綜合征，可有效改善患者白蛋白、24 h尿蛋白定量，降低腎功能指標(biāo)水平，還能調(diào)節(jié)血脂，改善患者血液流變學(xué)指標(biāo)水平，提高治療效果，臨床可推廣應(yīng)用。

Effects of Captopril Combined with Heparin on Renal Function and Hemorheology in Patients with Nephrotic Syndrome/WANG Yanqiu. //Medical Innovation of China， 2021， 18（05）： 0-044

[Abstract] Objective： To investigate the effects of Captopril combined with Heparin Sodium on renal function and hemorheology in patients with nephrotic syndrome. Method： A total of 92 patients with nephrotic syndrome admitted to our hospital from April 2018 to March 2020 were selected as the research subjects， and the patients were divided into control group and research group by envelope method， with 46 cases in each group. The control group was treated by routine treatment， and the study group was treated by Captopril combined with Heparin Sodium. The levels of plasma albumin， 24 h urinary protein， blood urea nitrogen， blood lipid， renal function， hemorheology and clinical treatment were compared between the two groups. Result： After treatment， the plasma albumin level of the study group was higher than that of the control group， and 24 h urinary protein group was lower than that of the control group， the differences were statistically significant （P<0.05）. The total effective rate of the study group was 87.0%， higher than 60.9% of the control group， the difference was statistically significant （P<0.05）. After treatment， the level of HDL-C in the study group was higher than that in the control group， and the levels of TC， TG and LDL-C were lower than those in the control group （P<0.05）. After treatment， Scr and BUN levels in the study group were lower than those in the control group， with statistical significance （P<0.05）. After treatment， the levels of fibrinogen， high and low blood viscosity， plasma viscosity and D-dimer in the study group were all lower than those in the control group （P<0.05）. Conclusion： Captopril combined with Heparin Sodium in the treatment of nephrotic syndrome can effectively improve the amount of albumin 24 h urinary protein in patients， reduce the level of renal function indexes， regulate blood lipid， improve the level of hemorheology indexes in patients， and improve the therapeutic effect， it can be popularized and applied in clinical practice.
