DRGs在精細化管理2型糖尿病患者中的應用

[摘要] 目的 探討DRGs在2型糖尿病患者精細化管理中的應用。 方法 通過信息軟件調取本院自2017年5月至2018年12月的2型糖尿病患者750例為研究對象。收集所有2型糖尿病患者的住院時間、藥占比等信息，利用臨床路徑、DRGs管理平臺，縱向比與歷史數據之間相比較，橫向比則與收治同種疾病其他醫生對照，與醫院制訂的各監測項目的目標值進行比較。觀察實施DRGs前后，2型糖尿病患者的醫療質量（平均住院時間、住院費用、藥占比等）情況、滿意度及血糖指標。 結果 在實施DRGs后，2型糖尿病患者的平均住院時間短于實施前，住院費用、藥占比低于實施前，差異有統計學意義（P<0.05）;實施后，2型糖尿病患者的服務總滿意度高于實施前，差異有統計學意義（P<0.05）;實施后，2型糖尿病患者患者的糖化血紅蛋白、空腹血糖明顯低于實施前，差異有統計學意義（P<0.05）。 結論 在精細化管理2型糖尿病患者中應用DRGs，可以有效實現提高該類患者的醫療服務質量的目標，讓患者真正體會到了醫院的公益性，值得在醫院中推廣應用。

Application of DRGs in fine management of patients with type 2 diabetes mellitus

[Abstract] Objective To explore the application of Diagnosis Related Group System（DRGs） in fine management of patients with type 2 diabetes mellitus（T2DM）. Methods A total of 750 patients with T2DM in our hospital from May 2017 to December 2018 were selected by information software. The information of hospitalization days and drug proportion of all patients with T2DM were collected. Using clinical pathway and DRGs management platform， the historical data of the patients were compared vertically. The patients were compared horizontally with the same kind of patients treated by other doctors， and the patients′ data were compared with the target values of the monitoring items established by the hospital.To observe the medical quality （average hospitalization， hospitalization cost， drug ratio， etc.）， patients′ satisfaction and blood glucose indexes of patients with T2DM before and after the implementation of DRGS. Results After the implementation of DRGS， the average hospitalization days of type 2 diabetes patients was shorter than before， and the hospitalization expenses and drug proportion were lower than before， the difference was statistically significant（P<0.05）. After the implementation， the patients′ total satisfaction with type 2 diabetes was higher than that before the implementation， the difference was statistically significant（P<0.05）. After the implementation， the glycosylated hemoglobin and fasting blood glucose of patients were significantly lower than those before implementation， and the difference was statistically significant（P<0.05）. Conclusion The application of DRGs in the fine management of T2DM patients can effectively achieve the goal of improving the quality of medical services for such patients， so that patients can truly realize the public welfare of hospitals， which is worthy of popularization and application in hospitals.
