Nursing Care for a Child with Allergic Rhinitis

背景

四歲男孩，因患過敏性鼻炎, 而在門診哮喘中心進(jìn)行過敏原測(cè)定。

Background

A 4 year old boy needed an allergen examination at the Asthma Center because of allergic rhinitis.

交流

護(hù)士：您的孩子患了過敏性鼻炎，請(qǐng)去檢查一下過敏原，看是什么原因引起過敏的。

患兒家屬：做這個(gè)檢查有沒有副作用？

護(hù)士：請(qǐng)放心，就象做皮試一樣，沒有副作用的。

患兒家屬：那要刺那么多的點(diǎn)是做什么的？

護(hù)士(指著抗原液)：這是從各種容易產(chǎn)生過敏的動(dòng)植物中提煉出來的抗原。孩子若有過敏的話，一接觸過15分鐘就可以看得出結(jié)果。

患兒家屬：有沒有毒？

護(hù)士：沒有的，我們已做了很多病人，很安全的。

(檢查后)

護(hù)士：結(jié)果出來了，您的孩子是對(duì)塵螨過敏，有三個(gè)(+)，還有霉菌是兩個(gè)(+)。

患兒家屬：什么？你把什么蟲和霉菌扎進(jìn)我孩子的身體里了？我問你有沒有毒，你說沒有，你這不是害人么。我要告你，要你賠償。將來我孩子有霉菌感染，我和你沒完。

護(hù)士：您搞錯(cuò)了，這是試劑，沒有毒的。您也看到我們的用量極其少，而且刺完馬上用棉簽吸掉余量，對(duì)孩子是絕對(duì)安全的。我們已經(jīng)做過上千例了，這和預(yù)防針相似，但比預(yù)防針用量還小得多。所以您不要擔(dān)心。

患兒家屬：真的不會(huì)有事？

護(hù)士：請(qǐng)您相信我。我同您一樣愛您的寶寶，同您一樣希望他安全、健康。現(xiàn)在結(jié)果出來了，我們一起來想辦法幫助寶寶脫敏、幫助寶寶把病治好，好嗎？

Interactions

Nurse: Your child has allergic rhinitis. Please take him to have an allergen test to find out what he is allergic to.

Child’s Parent: Does this examination have any side effects?

Nurse: Don’t worry. It just like a skin test and should have no side effects.

Child’s Parent: Why did you give so many punctures on his arm?

Nurse: (Pointed to the liquid antigen) This is the antigen extracted from animals and plants which may cause allergic reactions. If he is allergic to it, we shall see the result 15 minutes after he is exposed to it.

Child’s Parent: Is the antigen toxic?

Nurse: No. We have examined many patients. It is safe.

(After the examination)

Nurse: Here are the results. Your child is allergic to dust mites with three +++ and to fungus with two ++.

Child’s Parent: What? What have you injected mites and fungus to my child’s body? I asked if it was toxic and you said no. You are hurting him. I’ll sue you. You must pay for what you did. I will not forgive you if my child is infected by the fungus.

Nurse: You misunderstood. This is a reagent which is not toxic. The dose is very small, and any excess antigen was absorbed by the cotton tips right away. It is definitely safe for him. We have done it on thousands of patients. The examination is similar to an immunization shots and the dose we used is much less than that. So you need not to worry.

Parent’s Parent: Is it really safe?

Nurse: Please trust me. I love your baby and hope he is safe and healthy just like you. The test results are here. Now, we can try to help him overcome his allergies and treat his illness. OK?
