肝內膽管細胞癌的形態學CT分型及臨床意義

摘要：目的 對肝內膽管細胞癌的形態學CT分型以及臨床意義進行評價分析，為今后的臨床治療工作提供可靠的理論依據。方法 抽取在2013年1月～2014年6月我院收治的臨床確診肝內膽管細胞癌患者24例，對其CT掃描情況展開回顧性分析，評價形態學CT分型以及臨床診斷價值。結果 本組24例患者中，包括有腫塊型者10例，CT表現為肝內結節狀腫瘤，未出現周圍膽管擴張現象；管周圍浸潤型者5例，CT表現為沿著膽管縱軸生長的樹枝狀腫瘤，合并有周圍膽管擴張；腫塊合并管周浸潤型者7例，CT表現出肝內結節狀腫瘤，并且合并有膽管擴張；管內生長型2例，CT表現為單發、多發腫瘤結節位于擴張的膽管內。結論 經CT檢查可以對肝內膽管細胞癌展開準確的形態學分型，為臨床治療方案的選擇提供了可靠的參考依據，臨床價值顯著，值得關注。

The Morphology of CT Intrahepatic Cholangiocarcinoma Type and Its Clinical Significance

Abstract：Objective To make an analysis of the CT morphological classification and the clinical significance for intrahepatic bile duct carcinoma，to provide reliable theoretical basis for future clinical treatment.Methods Extraction in the clinical diagnosis of intrahepatic cholangiocarcinoma patients between January 2013 June 2014 in our hospital 24 cases，for its CT scan were retrospectively analyzed，CT evaluation of morphology classification and clinical diagnostic value.Results 24 cases of patients，including mass type 10 cases，CT manifestations of intrahepatic nodular tumor，does not appear the around bile duct expansion phenomenon；The infiltrating around tube type 5 cases，CT performance for dendritic tumor growing along the bile duct longitudinal axis，with around bile duct expansion；Mass with perivascular infiltration were seven cases，CT showed nodular tumors in the liver，and combined with bile duct expansion；The tube growth form in 2 cases，CT performance for single and multiple tumor nodules located in bile duct expansion.Conclusion The CT inspection can make accurate CT morphological classification of intrahepatic bile duct carcinoma，to provide a reliable reference evidence for the selection of clinical treatment，significantly clinical value，worthy of attention.

臨床上肝內膽管細胞癌來源于肝內末梢膽管到肝門部膽管上皮細胞的一種腺癌，流行病學調查結果顯示[1]，肝內膽管細胞癌的發生率相對較低，發病原因尚不明確，臨床、實驗室等各項檢測均無特異性，臨床早期診斷存在一定的困難，因此影像學檢查對肝內膽管細胞癌的診斷具有重要的意義。近幾年隨著CT技術的不斷發展和日漸成熟，在臨床疾病診斷和治療中發揮了重要的作用[2]。本次研究中出于對肝內膽管細胞癌的形態學CT分型以及臨床意義進行評價分析的目的，對我院收治的臨床病理確診肝內膽管細胞癌患者的CT檢查資料展開了回顧性分析，現匯報如下。……
