老年腰椎間盤突出伴不穩經后路減壓椎間融合內固定術治療的效果分析

[摘要] 目的 通過臨床手術，綜合分析在對患有多節段腰椎間盤突出癥患者進行治療的過程中，采用釘棒系統內固定以及腰椎后路減壓的辦法，并結合使用椎體間融合器植骨融合的手段，揭示了使用以上的手術方法對多節段腰椎間盤突出癥進行手術后的實際效果。方法 使用釘棒系統內固定、腰椎后路全椎板減壓、椎間盤切除加椎間融合器植骨融合內固定的手術方法，對84例患有腰椎鄰近多節段椎間盤突出癥的患者進行手術，隨機分為觀察組和對照組各42例。對照組施行單純減壓，觀察組施行經后路減壓椎間融合內固定術治療。并對其進行術后的臨床效果的回訪記錄與其術前的病癥及治療記錄做分析研究。結果 觀察組患者治總有效率（95.23%），在術后的療效要明顯優于對照組（73.80%），兩組比較差異有統計學意義（P<0.05）。結論 采用釘棒系統內固定、腰椎后路全椎板減壓、椎間盤切除術和融合器椎間植骨融合內固定術對于相鄰多節段椎間盤突出癥具有非常顯著的療效。

[Abstract] Objective Through the clinical operation， comprehensive analysis the process of multi segmental lumbar disc herniation were treated，Using the screw rod system fixation and posterior lumbar decompression method， and combined with the use of interbody fusion cage implant bone fusion method， reveals the actual effect of operation of using the above methods of operation for multi segmental lumbar disc herniation. Methods The use of screw rod system fixation， posterior lumbar laminectomy， discectomy with interbody fusion with bone graft fusion and internal fixed operation method， operation on 84 patients with multi segmental lumbar adjacent intervertebral disc herniation， were randomly divided into observation group and control group with 42 cases in each group. The control group received simple decompression，the observation group treated by posterior decompression of intervertebral fusion internal fixation surgery treatment. 椎間盤突出癥是脊柱退變性疾病的一種。該病最主要的原因是椎間盤的各個組成部分在其退變的過程中，受到來自外界不同程度的損傷而導致的椎間盤突出。而臨床治療中單節段椎間盤突出最常見，對其治療的方法已達成共識，保守治療無效后施行椎板開窗摘除髓核手術，簡單但是療效顯著。但由于多節段椎間盤突出癥在病理及生理改變等各方面都有很獨特的表現，而且涉及到生物學變化方面的知識，因此，對其治療要復雜得多[1-2]。……
