層級方案在血液透析中的護理效果評價*

【摘要】目的評價層級護理管理方案在血液透析中的應用價值。方法從2013年5月～2014年6月在南充市中心醫院血透室實施了護理層級管理方案，比較實施前后血透患者并發癥發生率、護理差錯或缺陷發生率、健康教育達標率、醫護人員及患者滿意度情況。結果血透室在實施層級管理后血透并發癥發生率、護理差錯發生率明顯減少，健康教育達標率、工作人員工作主動性及責任感明顯提高，患者和醫護人員滿意度均得到提高，差異均有統計學意義(均P<0.05)。結論層級護理方案的實施對血透護理水平、患者治療質量及醫護滿意度的提高均起到了積極作用。

Abstract【】ObjectiveTo evaluate the application value and effect of level management in hemodialysis nursing. MethodsNurses hierarchical management scheme was implemented in Nanchong Central Hospital hemodialysis room from May 2013 to June 2014. The incidence of complications, the incidence of nursing errors or defects, health education compliance rate and the health care and patient satisfaction situation before and after the implementation were compared. ResultsCompared with that of before the implementation, the complication rate and the incidence of errors in nursing were significantly reduced, the health education compliance rate, the staff work initiative and responsibility, patient satisfaction and the health care were significantly improved(P<0.05). ConclusionHemodialysis care level, patient treatment quality and the care satisfaction are enhanced by the implementation of the care hierarchical management programs.

護理層級管理就是根據護理人員的職位、職稱及實際業務能力等現實條件進行不同等級、不同層次的標準化和目標化管理[1]。通過建立層級管理模式，將護理人員按層級分布,充分利用有限的人力資源，激發護理人員工作責任心和工作積極性，從而減少疾病護理并發癥，避免差錯發生率，提高護理質量。

血透室是醫院一個特殊治療部門，治療患者多為門診病人，普遍遵醫行為較差，隨意性較大，且有病程長、需終生治療、病情復雜、變化快等特點。針對這部分患者，護理上需要相應地特殊服務。2005年衛生部《中國護理事業發展規劃綱要(2005~2010)》明確要求各級衛生機構要根據臨床崗位的工作職責和技術要求，調整護士隊伍結構，實施護士分層級管理[2]?！?br>
