高通量血液透析聯(lián)合左卡尼汀治療尿毒癥心肌病的臨床研究

【摘要】 目的：綜合分析高通量血液透析聯(lián)合左卡尼汀治療尿毒癥心肌病的臨床治療效果。方法：選取本院2014年3月-2016年10月收治的尿毒癥心肌病患者78例，按照不同的治療方法隨機(jī)分為試驗(yàn)組（高通量血液透析聯(lián)合左卡尼汀治療）與對(duì)照組（普通血液透析治療），各39例。兩組均連續(xù)治療7個(gè)月。觀察分析兩組治療前后的血常規(guī)指標(biāo)（血細(xì)胞比容、血紅蛋白）和心臟彩超結(jié)構(gòu)變化（左心室舒張期末內(nèi)徑、左心室收縮末期內(nèi)徑、射血分?jǐn)?shù)）以及生化指標(biāo)（尿素氮、血清肌酐和白蛋白）。結(jié)果：（1）治療前兩組血常規(guī)指標(biāo)比較差異均無統(tǒng)計(jì)學(xué)意義（P>0.05），治療后試驗(yàn)組血常規(guī)指標(biāo)均顯著高于對(duì)照組（P<0.05）；（2）治療前兩組心臟彩超結(jié)構(gòu)變化比較差異均無統(tǒng)計(jì)學(xué)意義（P>0.05），治療后試驗(yàn)組患者射血分?jǐn)?shù)增高，左心室舒張期末內(nèi)徑、左心室收縮末期內(nèi)徑均顯著低于對(duì)照組（P<0.05）；（3）治療前兩組生化指標(biāo)比較差異均無統(tǒng)計(jì)學(xué)意義（P>0.05），治療后試驗(yàn)組尿素氮、血清肌酐水平均顯著低于對(duì)照組（P<0.05），試驗(yàn)組白蛋白水平顯著高于對(duì)照組（P<0.05）。結(jié)論：高通量血液透析聯(lián)合左卡尼汀治療尿毒癥心肌病的臨床效果顯著。

【Abstract】 Objective：To analyze the clinical curative effect of high flux hemodialysis combined with Levocarnitine on uremic cardiomyopathy.Method：78 patients with uremic cardiomyopathy treated in our hospital from March 2014 to October 2016 according to the different treatment methods were randomly divided into the experimental group（high flux hemodialysis combined with Levocarnitine therapy） and the control group（hemodialysis therapy），39 cases in each group.Two groups were treated continuously for 7 months.Blood routine indexes（hematocrit，hemoglobin），echocardiography（structural changes of left ventricular end diastolic diameter，left ventricular end systolic diameter，left ventricular diastolic function，ejection fraction） and biochemical indicators（blood urea nitrogen，serum creatinine and albumin） of two groups before and after treatment were compared.Result：（1）Before treatment，the blood routine indexes of two groups were compared，the differences were not statistically significant（P>0.05）；after treatment，the blood routine indexes of the experimental group were significantly higher than those of the control group（P<0.05）.（2）Before treatment，echocardiography of two groups was compared，the difference was not statistically significant（P>0.05）.After treatment，the left ventricular end diastolic diameter，left ventricular end systolic diameter of the experimental group were lower than those of the control group，ejection fraction was higher than that of the control group（P<0.05）.（3）Before treatment，biochemical indicators of two groups were compared，the differences were not statistically significant（P>0.05）.After treatment，urea nitrogen and the serum creatinine level of the experimental group were significantly lower than those of the control group，and the albumin level of the experimental group was significantly higher than that of the control group（P<0.05）.Conclusion：The clinical curative effect of high flux hemodialysis combined with Levocarnitine on uremic cardiomyopathy is significant.endprint
