超聲與OCT在增殖性糖尿病視網(wǎng)膜病變中的診斷價(jià)值

【摘要】 目的：探討超聲、光學(xué)相干斷層掃描（OCT）診斷增殖性糖尿病視網(wǎng)膜病變（PDR）的臨床價(jià)值。方法：回顧性分析80例（156眼）增殖性糖尿病視網(wǎng)膜病變的超聲及OCT檢查，計(jì)算兩種方法陽(yáng)性檢出率，比較分析兩種方法單獨(dú)檢查及聯(lián)合檢查的檢出率。結(jié)果：80例（156眼）中，超聲檢出率為78.8%（123眼），OCT檢出率為55.1%（86眼），超聲對(duì)增殖性糖尿病視網(wǎng)膜病變檢出率高于OCT，差異有統(tǒng)計(jì)學(xué)意義（P=0.000），且超聲聯(lián)合OCT檢查檢出率明顯高于超聲檢查，差異有統(tǒng)計(jì)學(xué)意義（P=0.000）。結(jié)論：超聲與OCT對(duì)診斷增殖性糖尿病視網(wǎng)膜病變都具有重要的價(jià)值，兩者聯(lián)合使用可以顯著提高增殖性糖尿病視網(wǎng)膜病變患者眼底病變的檢出率。

【Abstract】 Objective：To investigate the clinical value of ultrasound and optical coherence tomography（OCT） in diagnose of proliferative diabetic retinopathyf.Method：Totally 80 patients （156 eyes） with proliferative diabetic retinopath（PDR）were evaluated by ultrasound and OCT examination.The detection rate of the two methods and the combined examination were compared.Result：78.8%（123eyes）were detected by Ultrasound，and 55.1%（86eyes）were detected by OCT.The detection rate of Ultrasound was significant higher than OCT， the difference was statistically significant（P=0.000），while ultrasound combined with OCT examination was significantly higher than ultrasound， the difference was statistically significant（P=0.000）.Conclusion： For proliferative diabetic retinopathyfultrasonography and OCT with advantages and disadvantages are both importantThe combination of the two methods can significantly improve the detection rate of Proliferative Diabetic Retinopathyf，which is worth of promoting.

糖尿病視網(wǎng)膜病變（DR）是糖尿病最常見(jiàn)、最嚴(yán)重的并發(fā)癥之一，具有致盲性和不可逆性，其患病率達(dá)到6.3%[1-2]。對(duì)糖尿病患者進(jìn)行定期的眼底篩查可及早發(fā)現(xiàn)視網(wǎng)膜病變并防止不可逆性視覺(jué)損害的發(fā)生。光學(xué)相干斷層掃描（OTC）可精確測(cè)量黃斑區(qū)視網(wǎng)膜厚度及形態(tài)改變，近年來(lái)應(yīng)用于臨床篩查及復(fù)查隨訪，而對(duì)于屈光間質(zhì)混濁的增殖型糖尿病視網(wǎng)膜病變患眼，超聲檢查具有重要的不可代替的診斷價(jià)值，本文旨在探討OTC、超聲及二者聯(lián)合檢查對(duì)增殖型糖尿病視網(wǎng)膜病變的診斷價(jià)值。
