【摘要】 目的 應用三維超聲斑點追蹤技術(3D-STE)評價原發性高血壓(EH)患者[左心室射血分數(LVEF)正常]左心室扭轉運動及其臨床應用價值。方法 76例EH患者, 按Ganau等左心室幾何構型分類法, 依據左室質量指數(LVMI)、相對室壁的厚度(RWT)不同分為左心室構型正常(LVN)組
(28例)、左心室向心性重構(CCR)組(22例)及左心室向心性肥厚(CCH)組(26例), 同時選取30例健康體檢者作為正常對照組。比較四組研究對象LVEF、舒張末期左室后壁厚度(LVPWTd)、LVMI、心尖旋轉角峰值(Prot-A)、心底旋轉角峰值(Prot-B)、整體扭轉角峰值 (LVtw)和整體旋轉角峰值(LVrot)。
結果 CCR組、CCH組患者LVPWTd、LVMI高于正常對照組, 差異具有統計學意義(P<0.05);LVN組LVPWTd、LVMI與正常對照組比較差異均無統計學意義(P>0.05);LVN組、CCR組、CCH組LVPWTd、LVMI呈遞增趨勢, 比較差異具有統計學意義(P<0.05);四組LVEF比較差異無統計學意義(P>0.05)。與正常對照組比較, LVN組、CCR 組和 CCH組Prot-A、Prot-B、LVrot及LVtw均顯著增強, 差異具有統計學意義(P<0.05);LVN組、CCR 組和 CCH組患者Prot-A、Prot-B、LVrot及LVtw呈增加趨勢, 比較差異均有統計學意義(P<0.05)。結論 3D-STE 通過評價正常構型及左心室重構的EH患者左心室扭轉動力學特征, 反映左心室收縮功能的改變, 指導臨床早期干預, 延緩高血壓心臟病的病程進展。

【Abstract】 Objective To evaluate the left ventricular torsion dynamics in essential hypertension (EH) patients [with normal left ventricular ejection fraction (LVEF)] by three-dimensional speckle tracking technique (3D-STE) and its clinical application value. Methods A total of 76 EH patients were divided by different left ventricular mass index (LVMI) and relative ventricular wall thickness (RWT) of Ganaus classification of left ventricular geometry into normal left ventricular (LVN) group (28 cases), left ventricular centripetal remodeling (CCR) group (22 cases) and left ventricular centripetal hypertrophy (CCH) group (26 cases). Concurrent
30 healthy physical examination cases were taken as normal control group. Comparison were made on LVEF, left ventricular posterior wall thickness of end-diastolic (LVPWTD), LVMI, peak apical rotation (Prot-A), peak basal rotation (Prot-B), left ventricular twist (LVtw) and left ventricular rotation (LVrot) between four groups.
Results CCR group and CCH group had higher LVPWTd and LVMI than normal control group, and their difference was statistically significant (P<0.05). LVN group had no statistically significant difference in LVPWTd and LVMI, comparing with normal control group (P>0.05). LVN group, CCR group and CCH group had an increasing trend of LVPWTd and LVMI, and the difference was statistically significant (P<0.05). Four groups had no statistically significant difference in LVEF (P>0.05). Compared with normal control group, LVN group, CCR group and CCH group had enhanced Prot-A, Prot-B, LVrot and LVtw, and their difference was statistically significant (P<0.05). LVN group, CCR group and CCH group had an increased tend of Prot-A, Prot-B, LVrot and LVtw, and their difference was statistically significant (P<0.05). Conclusion 3D-STE can reflect the changes of left ventricular systolic function by evaluating the left ventricular torsion dynamics characteristics of EH patients with normal conformation and left ventricular remodeling, so as to guide early clinical intervention and delay the progression of hypertensive heart disease.endprint

EH是嚴重危害人類健康的最常見的心血管病, 其是我國腦卒中及心臟病最主要的危險因素, 而且還處于發病率、死亡率高和知曉率低的嚴峻局面。EH患者因心臟負荷不斷增加, 血流動力學及神經體液發生異常, 可致左心室結構重構[1, 2]。以往研究表明:預測高血壓預后的強有力因素是左心室收縮功能, 及早發現、及早干預, 可顯著降低或延緩高血壓心臟病的發生, 對高血壓患者尤為重要。3D-STE可采集左室三維全容積數據, 并在采集的容積數據內個跟蹤各聲學斑點, 實時追蹤心肌的運動, 可以及時有效識別高血壓患者左心室收縮功能的早期改變[3, 4], 獲得高血壓患者心肌早期受損的有用可靠信息, 是獲得高血壓患者左心收縮功能早期變化的一種全新的無創性操作檢測手段。……
