雙源CT血管成像在診斷頭頸部血管疾病中的臨床價值

摘 要：目的 分析西門子雙源螺旋CT血管成像在診斷頭頸部血管疾病中的臨床價值。方法 選取我院2017年12月～2018年4月179例高度可疑頭頸部血管性疾病患者，進(jìn)行雙源螺旋CT血管造影，應(yīng)用MPR、CPR、MIP、VR對西門子雙源螺旋CT血管造影成像，分析成像結(jié)果。結(jié)果 69例為動脈粥樣硬化，其中腦動脈硬化62例，頸動脈硬化7例；血管狹窄或閉塞：頸動脈70例，椎基底動脈39例，鎖骨下動脈42例，大腦前動脈18例，大腦中動脈18例，大腦后動脈24例；45例血管發(fā)育變異，33例為動脈瘤，1例為煙霧病，1例動靜脈畸形，前后交通動脈開放分別為80例和55例，腦動脈正常者10例，頸動脈正常者34例。西門子雙源螺旋CT可清楚顯示病變的數(shù)目、大小、部位、形狀以及與其周圍血管和相鄰顱骨的主要關(guān)系，與數(shù)字減影血管造影檢查及手術(shù)所見結(jié)果大體一致。結(jié)論 雙源螺旋CT血管成像在頭頸部血管病變中具有極大的臨床應(yīng)用價值，其檢查安全性較高、精確性良好，而且有掃描速度快，檢查過程短等優(yōu)點(diǎn)，同時大大降低了輻射劑量和對比劑用量，所得圖像能夠滿足診斷需求，值得臨床在頭頸部血管病變的篩查、診斷及治療后隨訪中首選應(yīng)用。

Abstract：Objective To analyze the clinical value of Siemens dual-source spiral CT angiography in the diagnosis of head and neck vascular diseases.Methods A total of 179 patients with highly suspicious head and neck vascular disease from December 2017 to April 2018 were enrolled in our hospital.Double-source spiral CT angiography was performed.The angiography of Siemens dual-source spiral CT was performed using MPR，CPR，MIP and VR，and the imaging results were analyzed.Results 69 cases of atherosclerosis，including 62 cases of cerebral arteriosclerosis，7 cases of carotid atherosclerosis；stenosis or occlusion：70 cases of carotid artery，39 cases of vertebral basilar artery，42 cases of subclavian artery，18 cases of anterior cerebral artery，there were 18 cases of middle cerebral artery and 24 cases of posterior cerebral artery；45 cases of vascular developmental variation，33 cases of aneurysm，1 case of moyamoya disease，1 case of arteriovenous malformation，the anterior and posterior communicating artery arteries were 80 and 55，respectively，10 with normal cerebral arteries and 34 with normal carotid arteries.Siemens dual-source spiral CT can clearly show the number，size，location，shape of the lesions and the main relationship between the surrounding blood vessels and adjacent skulls， and the results of digital subtraction angiography and surgery are generally consistent.Conclusion Dual-source spiral CT angiography has great clinical value in the treatment of head and neck vascular lesions.It has high safety and accuracy， and it has the advantages of fast scanning speed，short inspection process and greatly reduced radiation.The dose and contrast dose，the obtained image can meet the diagnostic needs，it is worthy of clinical application in the screening，diagnosis and post-treatment follow-up of head and neck vascular lesions.
