三維超聲評價絕經前女性雌激素水平與肛提肌功能的關系

【摘要】 目的：采用三維超聲評價絕經前女性的肛提肌功能，并分析其與雌激素水平的關系。方法：選取2017年1月-2019年1月于本院進行健康體檢的60例女性。按照是否絕經分為對照組（絕經）和研究組（絕經前），各30例。分別采用三維超聲進行檢查，靜息時、Valsalva時分別測量肛提肌裂孔前后徑、面積、肛提肌長度，測定雌激素水平，并采用Pearson相關性分析肛提肌功能與雌激素水平的關系。結果：靜息時、Valsalva時，研究組肛提肌裂孔前后徑均短于對照組，肛提肌裂孔面積均小于對照組，差異均有統計學意義（P<0.05）;研究組肛提肌面積改變量小于對照組、雌激素水平高于對照組，差異均有統計學意義（P<0.05）;相關性分析顯示，經前女性雌激素水平與肛提肌裂孔前后徑、肛提肌裂孔面積、肛提肌長度、肛提肌裂孔前后徑改變量、肛提肌裂孔面積改變量均呈負相關（P<0.05），與肛提肌長度改變量不相關（P>0.05），絕經后女性雌激素水平與肛提肌各指標均無相關性（P>0.05）。結論：三維超聲能夠通過測量肛提肌裂孔前后徑、面積及肛提肌長度評價肛提肌功能，絕經前女性雌激素水平與肛提肌功能存在相關性，雌激素水平越低則肛提肌功能越差，絕經后女性雌激素與肛提肌功能不相關。

Relationship between Estrogen Level and Levator Ani Muscle Function in Premenopausal Women Evaluated by Three-dimensional Ultrasound/LI Xinxin.//Medical Innovation of China，2019，16（18）：-131

【Abstract】 Objective：To evaluate the levator ani muscle function in premenopausal women by three-dimensional ultrasound，and to analyze the relationship between levator ani muscle function and estrogen level.Method：A total of 60 women who underwent a physical examination in our hospital from January 2017 to January 2019 were selected.According to whether or not menopause，they were divided into control group（menopause）and study group（premenopause），30 cases in each group.Three-dimensional ultrasound was used to examine the levator ani muscle，the anterior and posterior diameters，area and length of levator ani muscle hiatus were measured at rest and Valsalva，estrogen level was measured，pearson correlation was used to analyze the relationship between levator ani muscle function and estrogen level.Result：At rest and Valsalva，the anterior and posterior diameters of levator ani hiatus in study group were shorter than those of control group，the area of levator ani hiatus were smaller than those of control group，the differences were statistically significant（P<0.05）.The area change of levator ani muscle in study group was less than that of control group and estrogen level was higher than that of control group，the differences were statistically significant（P<0.05）.The correlation analysis showed that the level of estrogen was negatively correlated with the anterior and posterior diameters of levator ani muscle hiatus，the area of levator ani muscle hiatus，the length of levator ani muscle hiatus，the change of anterior and posterior diameters of levator ani muscle hiatus and the change of levator ani muscle hiatus area（P<0.05），but not with the change of levator ani muscle length（P>0.05）.There was no correlation between estrogen level and levator ani index in postmenopausal women（P>0.05）.Conclusion：Three-dimensional ultrasound can evaluate levator ani muscle function by measuring the anterior and posterior diameters，area and length of levator ani muscle hiatus，there is a correlation between estrogen level and levator ani muscle function in premenopausal women，the lower the estrogen level，the worse the levator ani muscle function，there is no correlation between estrogen and levator ani muscle function in postmenopausal women.
