雙鎖定鋼板內固定術治療成人肱骨髁間骨折療效分析

[摘要] 目的 分析雙鎖定鋼板內固定術治療成人肱骨髁間骨折的臨床療效。方法 方便選擇2016年12月—2017年12月該院收治的成人肱骨髁間骨折患者76例，依據隨機原則將其劃分兩組，設為研究組（38例，雙鎖定鋼板內固定術）和參照組（38例，常規固定治療），對比手術用時、住院天數、治療總有效率和并發癥。 結果 研究組手術用時（118.24±25.97）min、住院天數（16.73±3.05）d短于參照組（132.95±31.46）min，（18.19±3.13）d，差異有統計學意義（t=2.222 8， 2.059 3，P<0.05）;研究組治療總有效率（97.37%）顯著高于參照組（88.95%），差異有統計學意義（χ2=6.175 8，P<0.05）。研究組情感職能（85.27±6.48）分、精神健康（87.95±5.71）分、生理職能（88.55±6.53）分、社會功能（87.54±5.06）分、精力（80.51±6.78）分和軀體疼痛（88.76±5.44）分等生活質量評分顯著高于參照組情感職能（67.72±6.49）分、精神健康（69.67±5.33）分、生理職能（66.88±5.88）分、社會功能（70.95±5.52）分、精力（73.38±6.45）分和軀體疼痛（72.19±5.48）分，差異有統計學意義（t=11.796 2，14.426 3， 15.201 9， 13.657 0， 4.696 7， 13.228 2，P<0.05）。隨訪后發現，兩組患者未出現嚴重并發癥，如：切口皮膚壞死、深部感染和尺神經受損。 結論 成人肱骨髁間骨折應用雙鎖定鋼板內固定術可獲取良好的固定效果，同時可加快康復速度，效果滿意。

[Abstract] Objective To analyze the clinical efficacy of double locking plate internal fixation for the treatment of adult humeral intercondylar fractures. Methods 76 patients with adult humeral intercondylar fractures in the hospital from December 2016 to December 2017 were convenient selected and enrolled. The patients were divided into two groups according to the random principle. The study group （38 cases， double locking plate internal fixation） and reference group （38 cases， conventional fixed treatment）， comparing the time of surgery， length of hospital stay， total effective rate of treatment and complications. Results The operation time （118.24±25.97） min and hospitalization days （16.73±3.05） d were shorter than the reference group （132.95±31.46） min， （18.19±3.13）d，the difference was statistically significant（t=2.222 8， 2.059 3， P<0.05）; the study group of the total effective rate of treatment （97.37%） was significantly higher than that of the reference group （88.95%），the difference was statistically significant（χ2=6.175 8， P<0.05）. The study group's emotional function （85.27±6.48）points， mental health （87.95±5.71）points， physiological function （88.55±6.53）points， social function （87.54±5.06）points， energy （80.51±6.78）points and physical pain （88.76±5.44）points， the grading quality of life score was significantly higher than the reference group's emotional function （67.72±6.49）points， mental health （69.67±5.33）points， physiological function （66.88±5.88）points， social function （70.95±5.52）points， energy （73.38±6.45）points and physical pain （72.19±5.48）points，the difference was statistically significant（t=11.796 2， 14.426 3， 15.201 9， 13.657 0， 4.696 7， 13.228 2， P<0.05）. After follow-up， no serious complications were found in the two groups， such as incision skin necrosis， deep infection， and ulnar nerve damage. Conclusion The use of double-lock plate internal fixation for adult humeral intercondylar fractures can achieve a good fixation effect， and at the same time accelerate the recovery speed with satisfactory results.
