豬尾導管與傳統胸腔引流管在肺葉切除術后的應用效果比較

【摘要】 目的：比較豬尾導管與傳統胸腔引流管在肺葉切除術后的應用效果。方法：選取2017年2月-2019年2月本院行肺葉切除手術治療的84例患者，根據簡單隨機法分為對照組與觀察組，每組42例。對照組術后采用傳統胸腔引流管置管引流，觀察組術后采用豬尾導管置管引流。比較兩組術后引流總量、引流管留置及術后住院時間，采用數字評分法（NRS）評估并比較兩組術后第1、2、3天睡前疼痛嚴重程度，比較兩組術后并發癥發生情況。結果：兩組術后引流總量比較，差異無統計學意義（P>0.05）;觀察組引流管留置及術后住院時間均明顯短于對照組（P<0.05）;觀察組術后第1、2、3天NRS評分均明顯低于對照組（P<0.05）;觀察組術后并發癥發生率明顯低于對照組（P<0.05）。結論：與傳統胸腔引流管比較，豬尾導管不但可縮短患者肺葉切除術后引流管留置及住院時間，還可明顯減輕患者術后疼痛和降低術后并發癥發生率。

[Abstract] Objective： To compare the application effect of pigtail catheter and traditional thoracic drainage tube after pulmonary lobectomy. Method： A total of 84 patients underwent pulmonary lobectomy in our hospital from February 2017 to February 2019 were selected. They were divided into control group and observation group according to simple random method， with 42 cases in each group. The control group was treated with traditional thoracic drainage tube catheter drainage after the surgery， and the observation group was treated with pigtail catheter drainage after the surgery. The total amount of postoperative drainage， drainage tube indwelling and postoperative hospitalization time of two groups were compared. The severity of pain at 1， 2 and 3 d after surgery in the two groups were evaluated and compared by numerical rating scale （NRS）. The postoperative complications of two groups was compared. Result： There was no significant difference in the total amount of postoperative drainage between the two groups （P>0.05）. The drainage tube indwelling and postoperative hospitalization time of the observation group were significantly lower than those of the control group （P<0.05）. NRS scores at 1， 2 and 3 d after surgery of the observation group were significantly lower than those of the control group （P<0.05）. The incidence of postoperative complications of the observation group was significantly lower than that of the control group （P<0.05）. Conclusion： Compared with the traditional thoracic drainage tube， the pigtail catheter can not only shorten the drainage tube indwelling and postoperative hospitalization time of patients after lobectomy， but also significantly reduce postoperative pain and the incidence of postoperative complications.

肺葉切除術是一種常見于胸外科的手術治療方法，不管是胸腔鏡下肺葉切除術還是常規開胸肺葉切除術，手術結束后均需予以胸腔閉式引流處理，有助于順利和徹底清除殘留在患者胸腔內的氣體及液體，并且盡快促進余肺組織的迅速復張[1-2]。傳統胸腔引流管在操作期間可對患者胸壁組織造成較大的損傷，患者術后疼痛不適感較嚴重[3]。近些年隨著快速康復理念的加強以及患者術后追求較高的生活質量，傳統胸腔引流管帶來的缺點受到醫務人員的廣泛關注，因此急需尋找一種減輕患者術后痛苦及創傷嚴重程度的胸腔引流裝置[4]。近些年豬尾導管逐漸應用于胸腹腔積液的引流治療方面，臨床效果較為理想[5]。目前豬尾導管在肺葉切除術后的應用研究報道較少，因此本研究擬比較豬尾導管與傳統胸腔引流管在肺葉切除術后的應用效果，現報道如下。
